Hi,

Breast lift without implants is certainly a commonly performed operation and, in many patients, yields fantastic results.  Provided you haven't lost too much volume in the upper portion of your breasts, and that your biggest problems are generalized sagginess of the breast skin and tissue, as well as droopiness of the nipple and areola, you are a great candidate for breast lift (mastopexy).  This basically involves making incisions around the areola, a vertical incision from the areola to the fold below the breast, and, in a few patients, an incision in the fold at the base of the breast.  Through these incisions, the breast skin and tissue is re-shaped, tightened, and lifted.

The only limitation is that, in many patients, the breast tissue which is lifted into the upper breast tends to fall somewhat after surgery.  Although the droopiness and generalized sagginess stay better, if you have flat upper breasts, that problem tends to come back.  The best way to combat that is with utilization of a small breast implant.
