Armando Soto, MD answers: Breast Augmentation a year post-Breast Lift?

I had a Breast Lift over a year ago while my husband was in Iraq, and was told I won't need a Breast Augmentation. I was a 36DD and am now a 36D, and I was told I had enough fullness.

Now, a year later, I am not happy because I am flat on the top part of my breasts and still have to adjust them in my bra often throughout the day. My doctor says now I will need an augmentation. Is this normal, and do I really need it? Why is it fair that I should have to pay for the full surgery now when I asked a year ago, and was told I won't need it? My husband and I are upset about me not being happy.  We have spent a lot of money, I have tons of scars, and I'm feeling uncomfortable.

I'm really sorry you're not happy- unhappy patients reflect poorly on our entire profession...

It sounds like communication before your surgery wasn't as good as it might have been-

It has been my experience that no matter how large a breast a patient has, in order to achieve a full, round appearance at the top of the breast, you need an implant. Having said that, if placing an implant will make the breasts larger than the patient would be comfortable with, the advantages and disadvantages of the options need to be carefully reviewed. You would be just as likely to be unhappy if you ended up with the shape you wanted, but your breasts were significantly larger than you wanted.

In the other hand, while I know I always do my very best to achieve what my patient desires, there are variables I cannot always control, like lifestyle, compliance with garments and medications, biology of healing, skin elasticity, etc...

The outcome of this kind of surgery, in other words, is not always completely predictable and that's why we usually explain to our patients that while we do our level best to make everyone happy, a guarantee of outcome cannot be offered and subsequent surgery may be required, with added expenses.

Because happy patients are so important to us, we usually will work with them to reach a reasonable solution. Talk to your surgeon openly and collegially (this is not a time to be confrontational or angry- your surgeon did their best and it just didn't work out)- I bet he/she can do something to make the experience and costs more acceptable to you.
