Breasts that undergo a reduction or lift will sag with time. How much depends on the technique used, age, skin quality, and time.
The Vertical reduction technique seems to be more durable in terms of maintaining projection although in the wrong hands it can create a rather unattractice breast shape. With the Wise pattern/inferior pedicle breast reduction technique, a technique that over 70% of surgeons use including myself, the skin between the nipple and inframammary fold will stretch over time.
Another term for this is bottoming out. When this happens, the nipples tend to look as if they are pointing upwards. I try to counteract this phenomenon by setting the nipple areolar complex a little lower (around 1 cm) during surgery to camouflage things.
