18 yrs old, no kids - What type of breast surgery do I need? Breast lift? I want breasts that have no sagging and no creases at all, not even cleavage. The size is fine, and I like the way they look with my arms up, besides the fact that the nipples are so crooked. Is it possible?

Breast Lifting for 18 Year Old?
Thank you for the question and picture.
Your picture demonstrates significant breast asymmetry and ptosis. At some point, breast lifting may be helpful to you. Ideally, you will be able to wait to have this procedure after pregnancies and/or when you are in a stable relationship. Otherwise, at your age, the scars that result from breast lifting tend to be prohibitive.

Cleraly you will achieve the majority of your concerns with a well fitted bra. Any of the goals you have expressed will require scarring. In my experience, most 18 yearl olds are not willing to accept this tradeoff. Given your future weight gain and weight loss, I would discuss deferring any surgery until completion of childbearing.

Breast lift
It looks like you would need a small breast lift because of the position of the areola on the breast. But, you cannot eliminate your fold and the breasts will drop a bit overtime.

Indications for a breast lift
From your photos it appears your nipples are too low relative to the rest of your breast, and a breast lift will correct that for you.
However, please note there will be incisions around your nipples and possibly down your breast skin. I recommend looking at photos of breast lift surgeries to make sure you are comfortable with the incisions.

Sagging can be corrected with a breast lift
Asymmetry of the breast can show up in different ways. The size or volume of your breast appears fairly even, thought the skin envelope reflected in the nipple position is not. Your right breast shows a nipple which is falling below the fold, what we call breast ptosis. Breast lift surgery for breast enhancement
By the appearance of your photo- your breast shape and position would be improved with a breast lift. I'm puzzled by your desire for no 'folds' or 'cleavage'- since you already have both, a breast lift procedure certainly won't take these features away. Thanks so much for including a picture - it really helps.
You've got asymmetry in the position of your areolas - the left is lower than the right. Both areolas appear low on the breast mounds. In order to raise both of the areolas, you need excision of some skin and repositioning of thebreast tissue (i.e. a breast lift).
