What to do about medical clearance?

Hi everyone! Had my pre-op today and everything is set for June 28th! Still nervous but my doctor eased my nerves quite a bit today! However, still stressed about one thing! Unfortunately, I have an insurance provider that doesn't allow my provider to give me medical clearance which I think is so stupid! Those of you from Cali should know which provider I'm talking about. Anyways, the family doctor my surgeon referred me to wants to charge me $1,500 for medical clearance? I'm sorry, but I think that's a little steep? So was hoping you all can give some suggestions or advice! My day is getting closer and closer and I'm getting desperate. If I can't figure out what to do I may have to cancel my surgery which I definitely don't want:(. Thanks! Good luck to all of you!
