Has Anyone Had Experience with Oxford Health Plans Covering Breast Reductions?

I am wondering if anyone has had BR covered by Oxford? Secondly, I am confused on what is the first step to take when trying to get a BR covered. I already have an appointment with a chiropractor on Monday because I know that my insurance requires it, but when do I need to call my insurance company and when do I need to see a plastic surgeon? Any help would be greatly appreciated.

A lot of insurance providers have criteria you can download online or request by phone. This explains when reductions are covered. I tried to check the Oxford site but it requires me to have their insurance to access the documents, so no luck there.
