Disappearing Eyeliner

I went less than two weeks ago and had upper and lower eyeliner tattoo'd on - about two days later all of it completely gone!!! I called the tattoo artist and she says it's either a "freak" accident - or because my iron was low. Has anyone heard of this before? & Yes there was ink and pain!!
