Accent vs. Titan for skin tightening

Just hoping someone maybe able to help me out. I'm 39 and I'm considering have a skin tightening treatment and wondered if anyone has done any research into which treatment is the most effective.

I have had a consultation for the Accent treatment but i can't say i was too convinced that the results will be particularly good. I've read some information on Titan and feel unsure if this would be a better option.

Can I ask where you got the consultation for Accent (can't find it). What did you think about it? How much was the Accent qouted price? THX to you AM!!

I believe that TITAN is much better in terms of both results and tolerability than any other skin tightening laser application.
I perform about 8 procedures a week on patients between 30-60 years old, and see excellent results.
If you're not a smoker or sun worshiper, TITAN started at your age can probably be done as a single yearly treatment. This preempts sagging and you may never need surgicl intervention.

I have similar questions regarding the effectiveness of one of these laser skin tightening procedures versus the LifeStyleLift. Have any of you considered and compared these? Which did you choose and why? How to they compare on results, costs, recovery, etc.

I've been hearing about the wonders of Fraxel but I'm skeptical since there's no long term statistics to substantiate the claims of no wrinkles and no botox is required after 3-4 treatments. Plus no pain and very little down time. Also it's twice the cost of IPL. Which would you recommend and what is the difference between the two?

Fraxel is a great technology, they have come out with a new Fraxel re:pair that is more aggressive and gets even better results with one treatment. Once again it is new, but worth investigating. If safe and effective it will be one of the greatest invovations in facial plastic surgery over the past 10 years (according to one of it's prototype university dermatology professors who annoucned the technology at the American Academy of Dermatology last week, as well as on NBC's Today Show.

No matter what treatment that you have, you will likely need some Botox in the future. That is unless you have the procedure (GLX) that destroys the nerves that causes movement, but this tecnology is new and it would probably be wise to wait and look what patients look like longer term. Good Luck.

Fraxel and IPL are not comparable in any way. IPL is meant to minimize and reduce pigmentation issues. As a side benefit, you would see a little bit of wrinkle reduction but that is not it's main goal or result. Fraxel on the other hand, was created to substantially reduce wrinkles, minimize pores, address pigment and tighten all in the one technology.

I am an owner of a laser business in the Phoenix area and have performed thousands of these treatments in the last 6 or 7 years. Both of these treatments are a little painful. I use topical numbing for the Fraxel but the IPL doesn't really require it because of the bipolar infrared technology that I use. The results from both is quite dramatic but different and it depends entirely on what you are trying to accomplish.

a girlfriend manges a medicalspa and she has tested the accent laser for getting rid of fat. she's really excited about the results. accent laser isn't like the fake roller treatments or skin suctioning techniques. i think accent may work on cellulite. can't say much about titan. i think it compares like thermaege. Ms66.

I have been researching fat technologies and skin tightening technologies for the past ten years. I am a facial plastic surgeon in Los Angeles (Encino), CA. Based on the findings of doctors and nurses whom I trust and have had both treatments, I believe that Accent XL is the best of the treatments (when performed properly). There is a new technology coming out of Israel called Ultrashape that sounds promising. I wait to find out more about it. I have not been impressed with the results or patient satisfaction rate with either Thermage or Titan.

This is very hard. You read so much, who do you believe, the only way to figure it out is to try it, but in this case the only thing you could lose is a lot of money. I see you offer Accent in your practice, so I question your good words. I wish more than anything to be able to trust people. I dont mind spending the money, I just want to receive the product/service i have paid for.

I'd be dizzy too if I had to go by all of the confusion out there. I'm sorry for it, but the reality is that everyone is trying to make themselves money, and unfortunately not everyone cares if they are giving the best treatment available to their patient. The laser companies want to sell their product, the practitioners want to sell their services to pay for the laser, and the uninformed patient gets stuck with being unsatisfied for their hard earned money.

As a physician, it is also dizzying for me to try and decipher which technologies are safe, effective, and not just hype. I do that by attending 4-6 national and international meetings/yr, keeping up on all of the facial plastic, plastic, and dermatological journals, speaking with colleagues whom I trust about their honest opinions on the technologies, "demo"ing the treatments on my own patients, and hearing what patients say about the treatments.

I incorporated Accent XL into my practice after five years of investigation. I have a twenty-two year reputation and am not willing to ruin it based on technology that does not satisfy my patients. My patients receiving Accent XL are very happy. I believe that it has to do not only with the Accent XL performs, but how the person applying the treatment does the treatment(I am confident that my patients will see excellent results, but I cannot predict how patients will feel in other offices. The technology works when it is performed properly).

I am 53 with some deep lines beginning to form around the nose and mouth, as well as the neck beginning to look scrawny. I started Accent laser a couple of months ago- have signed up for 6 treatments and have received 4 of them. At first, I began to see small differences, after the second treatment I got very excited that this would be the best thing since sliced bread. However, after the 3rd, it seemed to begin to reverse and lines began reappearing. After the 4th it got worse. I am afraid that if I continue with the last 2 treatments I will be back where I started. Is this unusual? The treatments are administered by a technician in a plastic surgery office in Boca Raton, who is an excellent physician with a great reputation. I would like an unbiased opinion as to whether a sort of reversal of beneficial effects is encountered in your practice at all. Thank you!

I usually advise that for "skin tightening," it is best to wait at least 1 month in between treatments, otherwise the collagen you are trying to stimulate is being re- broken down before it has had the opportunity to strengthen the integrity as a whole. I would wait and do the last two treatments about 6-8 weeks apart. It will help.

hi Jackster. I just had fraxel re:pair done last Monday...same as you...done on my entire face. So far so good. ;] Dr. Felder in Fort Lauderdale did mine and he does the mini lift you are int. in as well. He teaches/trains other Drs on laser and fillers. I love him. 954-771-1310.

Tara, thank you so much for the info. Actually I am having the repair done on my eyes tomorrow in Miami, Dr. Walder. She felt I didn't need to have my entire face done...I'm sure why, but she felt I would get more benefit with a mini lift that would address my jowling. Maybe I am too old for the repair to tighten enough!!! I called Reliant about a month ago to get the names of some docs in the area for repair and she was the closest to me, (Delray Beach). Hopefully all will go well, but I will definitely look into Dr. Felder for the mini.
How are you coming along with the repair? Are you happy with the results thus far? Please keep me posted and thanks again.

Hi, Tara it's me again. I just checked out Dr. Felder's site and he seems to do only injectable (short term) mini lifts. Very swollen and red, but they seem to get better by the hour. My doc is thrilled with how I am healing. I see her again on Monday and she wants to put some Botox on the sides to facilitate the healing.

Ok! Could a profeesional deliniate the difference between, refirme, thermage, Matrix IR, Elos, Titan, Accent. It seems like they are described as the same thing but different technology??? HELP. I am inquiring as a 35 year old with skin laxity under my eyes, resulting in crepey looking skin or a collection of fine wrinkles in the canal from my tear duct to the top of my cheekbone. What would be the best option for eyes???? Please help!

i am n ot an expert but i had your problem 1st i had i filler hydrafiil grade i under the eyes to give a bit of padding the i had 3 treatments of teosyk meso injected into the skin under the eye to imprve crepiness and it looks a lot better

Hi Heather,

It is very confusing (see the other entries above). For your age and problem I think that I would treat you with Fraxel re: pair, followed possibly by a small amount of Restylane under your eyes. Read other entries here and you will see some problems with loss of volume under the eyes with Thermage (it may have been improperly performed, but I would stay away from it under your eyes). Hello, My name is Heather and I'm 31 years old. I just had one treatment with the Accent XL in Dallas, Texas on the back of my legs and butt. How long I should wait before my next treatment and what should I be doing between treatments? I really didn't notice any difference and hope I'm not throwing my money away. Hi Heather,

We like for our patients to wait one month between Accent XL treatments. We tell our patients to maintain excellent hydration before and after the treatments. Obviously exercise and a healthy diet(lot's of antioxidants, see Jonny Bowden's "The 150 Healthiest Foods on Earth") are the absolute best things that you can do for yourself. Once again, I believe that the treatment is operator dependent, so I cannot universally comment on your treatment. It is important for the areas to be heated to a uniformly high enough temperature, followed by a very vigourous (almost painful) massage of the treated area. I have not had experience with Palomar 1540 Lux Lift. The company that manufactures Velasmooth has a new impressive machine called Velashape. I would choose Velashape over Velasmooth. So I have done a lot of research on the skin tightening, non surgical alternatives. I have talked with physicians regarding thermage, accent xl, combination IPL and accent XL. All this research just got me more comfused. I am 38 with some beginning jowls and would like some skin tightening, but do not want to waste money. What are your thoughts on accent xl and thermage, do they work, is it work the 3000.00 I am putting into it? thanks for the information.

I agree with you. Unfortunately, many companies and "laser providers" benefit from the confusion that is created about all of the different technologies. It is often times confusing even for physicians to make heads or tails over all of the claims made by companies and unscrupulous practioners.

My patients have had excellent results with Accent XL treatments on their necks and jowls. The treatment is done in my office by a very skilled RN who has a great deal of experience and who heats the skin to a specific temperature for a specific time, and then vigorously massages the area. The procedure feels like a hot massage, and is very tolerable. I can only vouch for the Accent XL technology in her hands, or in the hands of someone who knows what they are doing. I have not had good experiences with Thermage in the three patients I have personally treated, nor have my patients who have had the procedure elsewhere been enthusiastic about their results. It is very painful to boot.

I had 3 Titan treatments over the course of January and February of this year. The results have been ok i guess...I look a bit more refreshed but the improvement (especially on the loose skin on my neck) is not even close to what I'd hoped. I have an Accent session booked for next week and I'm wondering if its ok to go ahead with it from a timing point of view as well as using a different type of technology. I kinda like my face and I don't want to mess it up. Thanks!

Your TITAN provider should have told you that TITAN takes up to six months to work! What I mean is you will see very slight improvement at first, but it is NOT an overnight fix! All the materials put out by the maker (Cutera) are very clear that maximum improvements are seen from 3 to 6 months POST final treatment.

As you are only possibly one month from your last treatment I believe you are going to waste you money doing a different type of procedure without giving your yourself time for the TITAN improvements to develop.

Honest timelines, as well as realistic expectations are so important! Education, and information given at the consult, and then reiterated during the procedure is a must. Hope this helps :)

This is a question for Dr. Persky, since he seems to be the only physician following the blog. I am 27, and am considering Accent, but am not sure if it is appropriate for my concerns, or premature. I am mostly worried about the fine lines, soon to be not-so-fine lines, under and around my eyes, and have heard that Accent is a good way to diminish the appearance of those lines, and prevent them from becoming fixed, prematurely in my case (in my opinion). It seems that most of the writers to this blog are over 30, so I'm starting to think I'm just being paranoid and will seem silly walking into a dermatologists office asking for this procedure.

What would you say?

Hi Young (we'll leave out the Paranoid part),

As far as these types of questions go, we try to encourage patients to come in and be evaluated. You are NOT crazy, or too young, as this is when we all start seeing changes, some sooner than others. The accent is perfect for prevention, but we are not likely to recommend more than two treatments per year at this point. I often treat patients with 2 units of Botox placed just under the lower eyelid lash line for fine lines in that area. Botox also works well for the more lateral eye "crow's feet" wrinkles. If you choose to go with Botox, make sure that your doctor has experience with it in that area as I have seen some complications in patient injected by an internist. You can email photographs of your area of concern if you want me to give you a more detailed answer (as to your paranoia). Enjoy your youth; nothing we can offer in facial plastic surgery can beat it.

Hi Tom,

I am impressed with what I have seen with Velashape's before and after pictures. The Ultrashape (from Israel) should soon be released in the US; I think that it will be the next big thing in body shaping. I have no experience with Regen. Lastly, Alma laser has come out with a much larger hand piece head that makes it quicker and easier to get great results with the Accent XL. All of these technologies are very expensive so it is most important for you to do your due diligence before buying. Get a list of users of the product and contact them, see what their experience and thoughts are. Have the company let you treat a few of your own patients and follow their results. There are many products on the market, and it does seem that the more you research, the more confusing it becomes. Take you time and good luck.

Thank you for your candid answers to all the questions on devices. There is a new one called SmoothShapes that is coming out this year. It combines endermologie rollers, vacuum and lasers. Can you tell me if you think this technology would be similar or better than Accent or Velashape?

We currently have endermologie only and are ready to upgrade to a newer technology this year. As you know, these machines cost a lot of money and I would hate to invest in a failure for investment purposes and for my clients who trust my decision. I too am overwhelmed with RF, laser and ultrasound, and IPL since we have only used vacuum and massage techniques. I have always thought though that endermologie should "add" another modality to the existing massage. Now, a new company has done that. What do you think?

There are so many new technologies with so many "claims" and "promises". I agree with you that these technologies are very expensive and only a small percentage live up to their hype without becoming a very expensive clothes rack. I am not familiar with the SmoothShapes device, so unfortunately cannot comment. At the end of May and in mid June I will be attending 3 national meetings on aesthetics and facial plastic surgery. These meetings are usually my best source of new information that is most accurate. I will definitely keep my ears open and contact you if I hear anything.

When I am considering the purchase of new technology, I usually want to see the results with my own eyes, preferably by using the device on a few of my own patients. Then I try to find all of the research written about it (not research that has been sponsored by the manufacturing company of the device). If I am still interested, then I try to contact as many of the users of the device that I can and have candid conversations about its use. Again, it is essential to know practioners who are honest and with impeccable reputations. When in doubt, it is always best to wait. If the technology is that good, eventually the decision to buy will be a no brainer. Most importantly with new technology it is best to let others work the kinks out, make the mistakes, and protect your own patients from unnecessary risks and complications, after all it is about doing the best for your patients and making sure that what you offer is the real deal. can you have Accent skin tightening laser on your face when you have previously had fillers ? (Juvaderm and hyalauronic acid based fillers) I have been told this is fine but i am a little concerned as i cant help but think that the laser could break the filler down. As the Accent laser targets the water molecules and fatty deposits i have been told it will not affect the fillers. does anyone know ?

There have not been any studies on the effect of Accent XL on fillers. We do know that heat does break down hyaluronic acid. Some practioners use Accent XL to smooth out "lumpy" filler treatments. To be absolutely safe, you can wait until the filler disappears and then have the facial Accent XL treatments. Be well and good luck.

I am considering Accent XL on my neck and face area for tightening. I use micro-retin A and was wondering if that will make my skin more sensitve. Does anyone know if I should stop use before the procedure and if so, for how long?

We don't usually have anyone stop micro-retin A before Accent XL treatment. We have not had any bad experience given that it is not a light based therapy. Heat sensitivity might be more prominent, but shouldn't affect the outcome. Definitely discuss this with your physician or nurse BEFORE YOUR TREATMENT. Good luck and be well.

Hi Abbey,

It depends on what type and degree of laxity that your face and neck skin have. For very loose, thin lax skin, there are other modalities that are preferred (surgery, Fraxel repair, Deep Fx), while for thicker/fatter loose skin, Accent XL is an excellent treatment choice. If you are looking for just a little lift in the early jowling area, and overall tightening, then a combination of Accent XL with Fraxel re:pair will do the trick. As I ramble on I realize that it is difficult to make a recommendation for you without actually examining your face and neck. You can send a picture to my email if you want a more precise answer. Is there a difference between Titan treatments and Thermage?

Hi pallas,

They both tighten the skin by heating the subcutaneous tissues. They both have relatively low patient satisfaction percentages in the patients that I have spoken to. I have only used the Thermage technology in 3 patients, none of whom were satisfied. I have not tried Titan, but its rating on this site is even less then that of Thermage. When they work, they are great, but no one knows whom they will work on and whom they won't. Thermage should not cause underlying damage to the skin, though early on there were reports of fat atrophy from over treatment. Be well.

I just had my first accent laser treatment. Too early to tell yet how good the results will be. However, the technician that performed it used baby oil (and LOTS of it) as the lubricant and it made my face break out a bit. Does anyone have any experience with using an essential oil rather than baby oil? Is it mandatory that an oil is used?

Yes it is neccesary to use oil, however, the worst one on the face is baby oil. The best is grapeseed or pure almond oil, it wont clog the pores. Baby oil is fine for the body. Good luck and be well.

I am looking a treatment to help reduce waist circumference. I work out 5 days a week and have a relatively good diet. My waist looks out of proportion to the rest of my body. I have looked into liposelect, smartlipo, lippodissolve, velashape and now and reading about Accent. There are too many options. I think I have narrowed down to Velashape or Accent. Which one would show better results for the tummy area? Thank you for your help.

The non-surgical treatments of Velashape and Accent XL are effective in decreasing waist size. If you are physically fit and are having trouble getting those "last few spots of "pudding" then look into the Vaser lipo procedure which is perfect for those last stubborn areas. Also look for the release of Ultrashape (out of Israel) in the near future here in the U.S. Good luck and be well.

Hi.

Accent is a Radio Frequency (RF) device and Titan is a light based device, in my clinic we chose Accent. The laws of physics only allow light to effectively penetrate a small depth into skin, if your skin is darker then light has even less depth of penetration. Accent on the other hand uses RF which is not affected by skin color, it has two modes of delivering its energy (Bipolar and Unipolar) Bipolar only affects the superficial layers of skin and is good for skin tightening and improving skin texture. The Unipolar can penetrate much deeper and can actually remove small areas of isolated fat by a process know as Q10.

The principle of both technologies (light and RF) is to create heat in the deeper layers of skin, the heat then stimulates your own body into action to produce more collagen. So there are 2 factors that will determine your cosmetic result, the more heat that can be delivered effectively to the deeper layers of tissue and your own ability to produce collagen, (your age will greatly effect the latter). We chose Accent because RF can go deeper than light and heat more effectively.

I am 30 years old and looking for a procedure that will tighten the skin on my legs (all the way down to my ankles) and reduce cellulite. I'm pretty physically fit (5'3 and 122 pounds) but I suffer from bad genes in the leg region. My mother is thin but has loose, saggy, cellulite covered skin on her legs. Would you recommend Accent XL for me? Or, would it be best to wait for the UltraShape the Dr. P. mentioned?

Anyone with experience (professionally or having been in the same situation) is asked to respond!

I would recommend Accent XL or Velashape. Ultrashape is not yet available in the US, and I just heard that it's clearance by the FDA may be held up. There will be many other non-invasive fat and cellulite treatments coming to the market in the next few years, but the two treatments mentioned above are what I would recommend today.
