Revision Rhinoplasty Advice Needed? (photo)

I had nose reshaping done in Thailand in Nov 2010.Apparently from the medical report,the doctor did not diagnose or corrected my deviated septum so the nose appears in an S-curve frontal view.What are other nasal issues I need to be aware of and make sure are addressed during the revision.I am not sure if I can find a doctor here in Thailand who is highly experienced and skillful enough on western nose revision and may have to go to the US for this.The surgical report and pictures are attached.

A: Revision Rhinoplasty

Your photographs indicate that you would benefit from a revision rhinoplasty.  The areas that need to be addressed are the following:

1. Nasal hump correction - it appears you have a Polly-beak deformity

2.Nasal tip needs to be rebuilt probably with a combination of cartilage grafts and fat augmentation

3.Correction of the deviated nasal septum

4. Correction of the upper lateral cartilages separation from the nasal bones

The surgery was performed over a year ago, so you may safely undergo a revision, but you need an experienced plastic surgeon to perform the procedure.

Options For Nose That is Still Too Wide/fat After a Year? (photo)

Just after rhinoplasty my nose is fat, of course... But a couple things were done that will still make it wider permanently... This bridge was flattened... and, the tip was deprojected If it's still too wide after a year, might I do alar base reduction? Also, is it possible to give the bridge less of a 'flat' look without having the long recovery/swelling of a full rhinoplasty? I'd like to do whatever to make it more narrow without the long recovery. I can't handle a long recovery again.

A: Your nose is fine! Leave it alone!

Your nose is perfectly fine and your surgeon has done a very nice job.  You are a guy and you do not want a small thin feminine nose.  Your profile and your front view are nice.  Your picture shows you smiling and of course when you smile your nostrils will widen.  That is a normal thing.  Enjoy the results and don't give it any more thought.  Its a nice nose and your face is handsome.  Messing with it can make things much worse potentially.  Good luck

Was Told Just Leave Recent Broken Nose and that it had Fracture from Previous Rhinoplasty 7 Years Prior, Does that Seem Right?

Im a disabled vet & had Rhinoplasty 7 years ago at the Military Hosp.On Mon I fell on my nose it bleed for a hour the ER said the ct scan shows it broken.The plastic surgeon today said the fracture is from my last surgery & to leave it,VA wont pay due to budget.My nose/cheeks/eyes are swollen & Im in so much pain, I throw-up when I eat making the pain worse.can this be a fracture from the last surgery? I feel its broken & they dont want to pay for the surgery. I get married in 6 weeks. :(

A: Clinical exam best way to evaluate broken nose

Clinical examination is the best way to determine whether you need a closed reduction to straighten your fractured nose.  CT scan may reveal a nasal fracture but the bones may not be displaced (crooked).  It is best to wait until the swelling is down to determine if you need treatment.  There is about a ten day window before the bones become too healed to re-shape.
