I have tried almost everything to get rid of my severe acne, but nothing seems to work in the long-term, so I am starting to think I need to consider a more drastic solution. When is surgery an appropriate and effective acne treatment?
Acne surgery is not surgery as it is generally conceptualized.
Acne surgery involves the use of a comedone extractor, sometimes augmented by the use of an #11 scalpel blade. These instruments are used to open up closed comedones and extract its contents, or to extract the contents of open comedones (blackheads). Some skilled physicians and aestheticians can perform the latter procedure with perfectly placed cardboards.
Acne surgery is beneficial if you have a comedonal type of acne. Some would also argue, since the comedone is the first step to the development of a papule, pustule and cyst, that it is beneficial for all types of acne.
In good experienced hands, acne surgery is an excellent therapeutic tool. Done properly it should leave the face looking much improved the next day. The tracks and marks at the time of the procedure do resolve without scarring or indentations.
However, if you have major acne problems as you describe, you would be better off with more traditional treatments for severe cystic acne such as hormonal therapy, Accutane or one of the newer antibiotics. You should return to your dermatologist with these concerns.
