A: Lifting a heavy breast

Thank you for the photos.  Based upon the degree of laxity in your right breast and the nipple position it seems more likely that you will need a lollipop or full lift.  The diameter of your implants is wide and much larger (heavier) implants will drop more over time.  Rather than going with a wider implant you may wish to consider a more projecting implant with the same diameter, essentially going from moderate to high profile. This will give a more prominant upper pole.

 In situations such as yours  it is mainly the natural breast tissue that is drooping beneath the implant.  I will often reduce the natural breast volume because it is the natural breast that responds more  to gravity.  In a perfect situation you would have identical implant volume and identical breast volume.  If you reduce some of the natural tissue you will have the opportunity to do this and have more similar implant volume.

Be sure your surgeon has a comfort level with this as the blood supply to the breast can be more tenuous in a lift/reduction patient with an implant already in place.

Will I Be Happy with a Breast Lift Alone and No Implant?

I am a 36 yr old mom of 2, currently a D cup and happy with my size. My beasts are saggy and have lost all upper volume. My nipples don't point down, but they are low and definitely need to be moved up. The doctor I visited said I only need a lift, but I can't help but wonder if I will be happy with out a small implant. I would have an anchor scar and he said something about an "internal bra" so that they would be "perkier" and that is why I wouldn't need an implant.

A: Volume does not equate to perky

If you have enough volume, then you do not need an implant, and a breast lift should suffice to giving you a very natural aesthetic result. I disagree that an implant is required for a "home run".

On the other hand, you want to have an extended discussion with your plastic surgeon as to what exactly will "make you happy." I would look at pictures as to what a your surgeons breast lifts look like 6 months and 1 year out and also see what some results are with and without implants.

As for full anchor incision, it is also not absolutely necessary. There are techniques that can give long lasting beautiful results without the full anchor incision.

Will I Need Breast Lift with Implant Downsizing?

I had a breast augmentation over a year ago. I was an A cup, and got a moderate silicone 225cc. I'm very petite, so I didn't want to go that big. I still wanted to look kind of flat, but with little very little cleavage. So, I want to get smaller implants, but I don't want to get a breast lift. I'm only 26, and I'm hoping I won't need it. Will I need a breast lift? What implant size should I go for? Thanks.

A: Breast lift usually not needed with downsizing

The most important issue with downsizing is skin laxity and breast tissue laxity after implant removal. Most often, larger implant downsizing results in a smaller lift, but your situation is different. You will respond well with implant downsizing alone. Best wishes.
