Is a brow lift (forehead lift) better to raise sagging eyelids than a blepharoplasty?

A: Best way to treat extra eyelid sag

As we age, the upper eyelid skin begins to sag. Sometimes, the skin can even hang on or below the eyelashes. Cosmetically, this appearance closes off the eyes and makes you look older but functionally, it is heavy on the eyelid and makes your eyes tired particularly toward the evening.

The heavy eyelid can sometimes be due to extra skin on the thin upper eyelids but is also often due to droop of the forehead skin and brows. An expert plastic surgeon will be able to evaluate the difference and give you three options for treating the problem.

1. Upper Eyelid Blepharoplasty: To remove the skin from the upper eyelids

2. Brow (Forehead) Lift: To raise the position of the forehead skin and brow which will at the same time lift the extra skin off of the upper lid. Oftentimes, you'll find that the upper lid didn't have so much extra skin afterall.

3. Both

Most patients require only the upper eyelid blepharoplasty which can be performed in the office without general anesthesia.

Transconjunctival Blepharoplasty Vs Arcus Marginalis Release

Hello, I am a 21-year-old male who definitely wants to have my (minor) eye bags removed. I know they're caused by fat because when I squint or lay on my back they disappear. What would benefit me the most, a transconjunctival blepharoplasty or an arcus marginalis release? What are the differences between the two, and the pros and cons of either? Thank you.

 
A: The type of eyelid procedure required depends on your problem

The type of eyelid procedure needs to be be tailored to the patient. As an expert in facial and eyelid procedures, your surgeon should be able to give you the pros and cons of each procedure. I perform the following for the following reasons.

1. Transconjuctival Blepharoplsty - This is a procedure most appropriate for very young patients ( less than 30 ) who have the problem of fat bags but not so much extra skin and wrinkles.

2. Arcus Marginalis Release - This is a procedure most appropriate for patients with deep tear trough and flat anterior cheek descent or who have a very exposed orbital line.

3. Traditional Skin Blepharoplasty - This is the most common procedure and is most appropriate for patients with more skin excess and wrinkles who are over 35 to 40 years of age. With this procedure, I can remove the skin excess as well as remove any fat bulges.

5. Nonsurgical Blepharoplasty - This is a procedure I am performing more commonly. I do this procedure with fillers (Radiesse, Juvederm, or Sculptra). By filling in the malar cheek and camouflaging the orbital bone line, any extra lines get plumped out, the fat bags get camouflaged, and the orbital line is erased.

Alternative to Eyelid Surgery and Botox?

I cannot use too much Botox in forehead because of droopy eyelids but I am not ready for Eyelid surgery. I would, however, like a smoother forehead and eyelid lift. Any suggestions?

A: Botox can be given to women with " Droopy Eyelids "

Botox can be given to women with droopy eyelids if it is given by a qualified surgeon who is very knowledgeable of the anatomy in this area. The treatments need to be made above the level of the first horizontal wrinkle, about 1.5 centimeters above the level of the brow. Matter of fact, botox in my practice is often used to raise the brow if delivered in the lateral brow. You need to find a plastic surgeon who sees complicated issues on a daily basis.

Having said this, the most appropriate treatment might afterall be a cosmetic eyelid or brow procedure. While you might think you might not be ready for this procedure, in the right hands, it is one of the easiest procedures that we do.
