A: Treating deep wrinkles under the eyes - Non Invasive and Surgical Options

Deep wrinkles under the eyes can be due excess skin laxity or loss of volume under the eyes.  Your non-surgical options include some form of skin resurfacing such as peels or lasers (consider fraxel CO2) and/or the addition of fillers such as Restalyn or Juvederm.  In addition, you might get some additional deeper tightening using Ultherapy under the eyes.

If there is a heavy amount of skin laxity, then I would consider a lower lid blepharoplasty where some of the skin is directly excised and the incision hidden under the margin of the lid.  This will give you the most flexibility to remove the wrinkles.  Volume loss can also be addressed by fat repositioning or fat grafting.

Will Fraxel Laser Give Me Any Relief from Eye Deep Wrinkles?

Is my only hope to use an injectable filler for deep wrinkles, or is there a type of laser that can penetrate noticeable creases and lines around my eyes?

A: Fraxel, eyelid wrinkles, lasers and peels

When one uses any kind of laser, it, the laser, doesn't automatically know what to do. The doctor doing the procedure has to decide on what settings and how many passes to use. There are many variables in addition to the technical skill involved. The dual Fraxel laser is on the low side for getting wrinkle reduction. Exquisite care must be used to avoid injury to the eye itself when treating wrinkles on the lids. A fractionated CO2 (carbon dioxide) laser is one step up (made by many companies) and then there is the fully ablative CO2 laser, all of which have been shown to be effective; the stronger the laser, the better the results in most cases, but the longer the recovery. The regular Fraxel requires a series of sessions, whereas fractional and full CO2 can be done in one session. Chemical peels can also be done and vary in strength and results depending on the chemicals used, their concentration and the skill of the doctor.

Fraxel Restore Vs. Fraxel Repair

Is the Fraxel repair laser a new version of the Fraxel laser (restore) that is already out? I can't really find any information that explains how Fraxel repair is different from Fraxel laser. Does Fraxel repair do the same stuff that Fraxel

A: Fraxel repair vs Fraxel restore - Ablative vs non-ablative laser

Fraxel Restore is a non-ablative laser. By that I mean, it does not remove the top layer of skin(epidermis). It works at a 1550nm wavelength. Because it is non-ablative, recovery time is faster, although multiple treatments are necessary to achieve desired results. Recovery time is minimal.

Fraxel Repair is an ablative fractional CO2 laser, that works at 10,600nm wavelength. It achieves better results for skin rejuvenation, often in a single treatment. It does have a recovery healing time of 3-7 days

When I consult with patients, not only do we look at the patient's skin concerns ,but also their skin type ,and what type of treatment program they are looking for. Many patients want a one-shot treatment ,even though there is downtime, whereas others would rather have multiple treatment sessions with no downtime.
