I Had A Pulmonary Embolism 28 Years Ago. I Am Considering Having a Face Lift. Is There Still a Big Risk?

28 years ago I had a pulmonary embolism. At that time I was a heavy smoker and had traveled a long plane flight while feeling under the weather. I was hospitalized for about 2 weeks and took blood thinners for approx.6 months. I have had no other episodes or complications from this event and I quit smoking at that time. I am now 67 years old and am considering a 4-5 hour elective surgery for a face lift. Am I still at risk for complications because of the previous pulmonary embolism event?

A: Facelift and risk of pulmonary embolism

A surgical procedure lasting four or five hours has an elevated risk of deep vein clot.  In patients with a previous history of deep vein thrombosis (DVT) resulting in pulmonary embolism the risk is increased further.  Calculating risk of another episode is difficult.  Options that decrease risk of DVT include:

  1. Local anesthesia with mild sedation.  Dramatically lower rate of DVT
  2. Shorter procedures and avoid combining procedures (like Blepharoplasty)
  3. Sequential compression devices until completely able to walk normally
  4. Low molecular weight heparin

Complete medical clearance and evaluation of heart and lung health is highly recommended.

Are Facelift Compression Garments offered with Natural or Synthetic Fibers?

For sensitive skin, will compression garments irritate skin? Are compression garments worn around the clock, and if so, for how many days?

A: Compression garments after face lift

Most garments are made not to irritate any skin. If the garment is irritating, fluffed gauze such as Kerlix can be placed between the skin and the garment. Often compression dressings are not placed initially but after the first dressing is removed. They come in all sizes and types from simple slings to total facial/neck garments. The rationale for use is to keep the skin in close approximation with the underlying structures so it will re-adhere quickly and not develop fluid underneath. Follow your surgeon's suggestions as to how long to wear it as there are many types of garments with different indications.

I Had a Facelift 4 Years Ago and Don't Want Another One What Can I Do to Help Sagging?

I had a facelift approximately 4 years ago and have started to get the deep fowls around my mouth. I'm 60 years old and want to look vibrant for my age but do not want a another facelift. Are there other alternative that will help the wrinkles and jowls?

A: How long do face lifts last? What can be done before a second facelift?

Most traditional complete facelifts generally keep you looking younger and refreshed for 7 to 10 years. You are always ahead of the game if skin has been removed and if muscles and fat pads have been resuspended. But, alas, the aging process proceeds, and, in every additional decade, we age more. In the interim between lifts, laser skin resurfacing  can reduce wrinkles and can tighten the underlyting skin; Thermage (radio-frequency) might help shrink the tissues as might Ulthera (an ultrasound technology). Simple soft tissue fillers like Restylane, Juvederm, or Radiesse can fill in fold and wrinkles.
