I understand and have heard that silicone, goretex and all other synthetic implants for rhinoplasty will eventually have to be replaced (or revised) after years of wear and tear. From this forum I have heard many surgeons recommending that the best material used for rhinoplasty is our own autologous tissue such as rib cartilage or septal cartilage. I would like to know if autologous tissue used for rhinoplasty will eventually require replacement or revision too?
When nose surgeons need to raise the roof of the nose they consider various materials as potential onlay grafts including Man Made implants (Goretex, Silastic, Porex / Medpor etc), other humans (cadaveric rib) or your OWN body (septum, ear cartilage or rib). Unless the graft is penetrated by your own blood vessels and becomes an integral part of your body, as your own tissues will, eventually they will become infected, coveredby hard scar tissue or be pushed through the skin and become extruded. Although ALL grafts can suffer these complications, the likelihood of tissue grafts from your own body having to be removed is much lower.

Hi there, I had Rhinoplasty on the bones within my nose to narrow them from front view and promote an S shape from side view,however, my tip was also very swollen after the op. I had the op ten months ago in Prague, I am from the UK. The tip is still swollen and i believe that the sides of my nose may also be swollen but am not sure the bone still appears quite wide. The skin here feels slightly soft and springy, could this be swelling, is the skin still contracting over my nose to shrink
At ten months you are looking at the nearly complete final appearance of your nose. Moreover, when the nose bones are in-fractured to narrow the nose especially when combined with lowering a nasal hump, the nose skin has to be somewhat peeled off the nose to allow this profile smoothing.
Prague is a beautiful city and I understand my colleagues there are cheaperthan my British colleagues (especially the Harley Street crowd) may be but you now have the classic dilemma of Surgical Tourism; You have a condition which your surgeon should answer and yet, yours is quite a ways away.
If you cannot see your surgeon, you may want to bite your lip and pay a good local nose surgeon to see you in face to face consultation to best advise you.

I live out of province from where i'm traveling to have my rhinoplasty done (24 hour drive). Is it a big deal to have the surgery, drive home, and have another surgeon in my home town do my post op (take off cast, make sure everything's ok)? Gets very expensive otherwise.
In the age of Medical Tourism where people travel to all corners of the earth in search for Cosmetic Surgery deals your question is extremely relevant and worth discussing.
I seriously doubt that your hometown Plastic surgeon would enthusiastically accept taking care of you after Cosmetic Surgery done elsewhere. No one likes to be passed over. If he was not good enough to perform your surgery, why would he be good enough to take up your aftercare? Worse yet, if your surgery encountered any complication, why would any local surgeon take a chance of being sued by getting involved in your care?
Before you go elsewhere for your surgery consider who would care for you after surgery including if you suffered any complications. All of this must be discussed and agreed with your surgeon before you go to the operating room.
