Can I Have a MACS Lift After Having a SMAS Lift?

About six months ago I had a SMAS lift which resulted in more overall laxness, asymmetry and a loss of definition in my cheekbones. My doctor called his procedure a "mid-face lift" and a "cheek lift." Can a MACS lift correct any of this? After further reading the MACS sounds like a much better fit for the results I am looking for, or is it too late?

A: Revisions of facelifts: Mini-lift, Cheek-lift, MACS, SMAS, mid-face lift. It's the technician, not the technique.

Dear Jean1962:  Yes you can have a MACS lift after another type of SMAS lift.  Experienced plastic surgeons use a variety of techniques when revising a prior facelift; MACS is an excellent technique.  However in the situation you describe, I doubt that it would be in your best interest to consider another facelift surgery so soon after a prior lift.  Your photo reveals some nice facial features; the items of concern should be discussed with your plastic surgeon and alternatives considered.  Injectable fillers or fat grafting may be good options at this point.

I think you should not focus on choosing an specific surgical technique, but rather focus on the recommendations of a trusted, experienced surgeon.  How your tissues are handled by your surgeon is more important to your good outcome than a particular incision, dissection, or suspension method he or she might choose.  "It's the technician, not the technique."  Best of luck.

Can Long Term Results Be Expected from Multiple Facial Surgeries?

I am scheduled for a facelift, brow lift, eyelid lift, CO2 laser, fat injections. I am 49 years old and very scared of the surgery itself. How long will the results last? Will I always look about 10 years younger? Or will I waste my money and assume the risk for something lasting only a couple of years and needs augmentation with fillers, Botox etc? Can anyone please help? I am in a panic and running out of time.

Most patients that consult me have been carefully considering facial cosmetic surgery for some time, and doing their research. Selection of an experienced facelift, etc. surgeon is very important. The surgeon and their office staff should have provided you with plenty of information; first, in order to decide on surgeon and procedures, and second, to reduce your fears by helping you understand the recovery and care you'll get.

Your face, brow & eyelids will typically be changed permanently by the procedures you mention. They will likely always look better than if you had never had the surgery, but surgery will not stop future skin and deep tissue aging. Genetics, sun exposure, smoking history and other factors affect the duration of surgical results. You should not be surprised that you would consider future procedures in order to maintain future results.

Factors to Consider when Deciding for a Facelift?

I am considering a facelift, age 56. Excellent health. Two of the four consults I have had prefer IV sedation, the other two, general. The estimated time for the surgery is 4.5 hours. This is consistent among all surgeons. The costs is approximately $3000 difference for general. I have been reading pros and cons to both. What factors should I consider in making my decision?

Since you have consulted with 4 surgeons, you are obviously doing reasearch in person. As for those secondary considerations:

(1) costs should be appropriate for the surgeon's results, the facility used, the staff's experience, etc.;

(2) the anesthesia method is that which your surgeon chooses to provide you his best results; and

(3) the facility should be accredited, and preferably regularly doing cosmetic surgery cases.

In our situation, we prefer our office aesthetic surgery facility (nationally accredited), with general anesthesia provide by a board-certified MD anesthesiologist that works specifically with us. We feel this combination gives our patients the best safety and care.

First and foremost, you should pick the surgeon you feel will most likely give you the best facelift result (both based on the techniques and the experience of care).
