QUESTIONS ANSWERED

Do I have Gum disease? ((photo)

Asked By:AnonymousANSWERS (1)

You do have some inflammation on your lower teeth, this could be just gingivitis which is inflamed gums only, which would resolve after scaling.

It is better to see a Periodontist for an exam and xrays to rule out bone loss and Periodontitis (which is a more advanced gum disease situation than gingivitis).

The black line that you see is pigmentation and is normal

How can I fix my loose gums? (Photos)

Asked By:AnonymousANSWERS (1)

From the picture you are providing, your gums look inflamed.

You will need to see a Periodontist for an exam and xrays from your teeth, to evaluate possible bone loss and treat accordingly.

You will need to consult with a periodontist.  He/ She will take xrays from these teeth to evaluate amount of bone loss (possibly the cause of your recession and the shift in your teeth).  are these teeth mobile?

What is you bite like? are you putting too much pressure on these teeth?

These are some of the factors that will need to be evaluated before deciding if these teeth could be saved or not, and if the gum recession could be corrected or not .....

It is not always possible to regenerate 100 % of the missing gum and cover all of the exposed root.

In these cases, after the gum graft is completely healed,  you might need white colored resin filling to cover the exposed root and to reduce the root sensitivity if any.

Sometimes some of the graft will get exposed and starts sloughing off,

but if the site is not disturbed , you will get good results

Please follow post op instructions provided to you by your surgeon.

In this case, there is spacing in between your teeth as well

your options is closing the contact in between the teeth preferably with  Orthodontic treatment treatment (one of your teeth is slightly also protruded) ,

or with a restoration ....

see your dentist to evaluate your teeth and to make sure not mobile due to bone loss.

It all depends on the amount of bone that is present.

- Immediate implants could be placed at the time of extraction.

- Implants could be placed as early as 8 weeks post extraction, this is the time that is needed for soft tissue healing.  At this time, some bone graft could also be added to bulk up the site if needed.

- In cases that the bone is very limited, ridge augmentation surgery will be needed , and then an implant could be inserted at 6-9 months post bone grafting.

To gain the best symmetry you will benefit from gum lift (gingivectomy / esthetic  crown lengthening)  all the way to your back teeth. Even though you are not going to veneer them ...

This is because in your profile pics You have a gummy smile which extends to the molars .....

A picture will be helpful.

There are different causes to gummy smile (excess maxilla/ excess bone/ excess gum) .  Each of these scenarios need to be treated differently.

you will need an exam/ xrays to diagnose and treatment plan.

Depending on the level of bone around your teeth, we use different procedures to address gummy smile.

If the bone level is an issue (too much bone), the results of using laser or blade to remove only the excess gum , will be temporary.  we will get a rebound and tissue re-growth even if you get re-treatment.

You might benefit from Esthetic crown lengthening (re-contouring of tissue and bone). After the healing from this initial surgery, if you are looking for more improvement, then you could look into lip repositioning/ or even Botox to limit the lip movement when you smile ....

Causes of gum recession are:

- Thin tissue (genetic)

-Frenum pull

- aggressive brushing

-smoking and gum disease

-clenching and grinding

- Orthodontic tx

*** your front 2 teeth are longer than the ones on the sides (centrals are longer than the laterals).

In your case, the gum recession is most likely, multi factorial (bite related, Ortho, frenum pll)

Have a Periodontist evaluate your teeth/ your bite and your gums.  An xray will be needed to evaluate the level of bone in between these teeth....   if possible, your teeth could be adjusted in height to reduce your bite pressure, they could be splinted to reduce the mobility, and then gum graft procedure will be needed to regenerate the tissue to prevent further recession and bone loss

Great Experience: Highly Recommend Dr Elly Tehrani

Bloor West Village Periodontics provided exceptional care throughout every step of my treatment. Dr Elly Tehrani and her team made me feel comfortable and very well cared for during teeth removal and bone graft. This was my first dental surgical experience so I was nervous going in. But Dr Tehrani explained everything clearly, conveying deep expertise and experience, which was highly reassuring. Thanks to Dr Tehrani and her team, my surgery & recovery was smooth and successful. I highly recommend Dr Tehrani for anyone considering oral surgery and dental care.

2 year journey as their patient

The most professional and expert clinic I've been to. Dr. Tehrani and Dr. Iskander are both so knowledgeable, adept and advanced in their practice and they always work to get things right and of the best quality the first time. From the get go they have been open, honest and balanced. They communicate the risks (e.g., the bone graft not taking), the alternatives (e.g., a dental bridge) and the cost vs benefit. Over 2 years, Dr. Tehrani and Dr. Iskander were able to install a dental implant in place of my missing front tooth, a sensitive and complicated case that involved a detailed radiologist report (outlining the need for a bone graft and the optimal placement of the implant), a bone graft which took, a scrape and root canal, advanced imaging for the crown and a number of pre-op and post-op checkups to ensure that the site and implant were healthy. I'm not in the profession but I've learned more than I anticipated about periodontics, all the risks of the procedures and all the subtle indicators they look for to assess the health of the site. Both Dr. Tehrani and Dr. Iskander are very personable and a pleasure to speak to and get to know. All the staff are so easy going and caring, even the receptionists, whom are my favorite! They always tried to accommodate me over this 2 year journey from something as small as seeing me earlier when I was in a rush to rescheduling an operation when I forgot to fast beforehand. They even replaced the lingual wire (from braces) on my lower teeth when it broke. Over the two years every appointment I had got better and better as I got to know them and they got to know me. Dr. Tehrani and her staff always check in with me to see how I'm doing, but I began to notice that they do this with everyone else. They care about the patients and the work they do. I'm glad I got to be their patient.

Thorough and Attentive Period!

I had to get a gum graft on my front two teeth. After doing extensive research, I decided to go with Dr. Tehrani. For a pretty uncomfortable procedure, she made me feel comfortable throughout the process and the office is so clean and bright. Whenever I had any questions, she took the time to answer and explain why, what, etc in great detail. I really appreciated that she explained things in depth without treating me like an idiot (like most in the dental field these days). Couldn't recommend her and her office enough! Dawna is also super nice!

Explains everything to you in full details

Dr. Tehrani is by far the best periodontist. I went to another one because of scheduling but he didn't come close to the work Dr. Tehrani does. She is very skilled at this surgery and explains everything to you in full details. You will not be disappointed at the work. Plus her office staff are so nice and accommodating.

Dental bone graft, 1st time seeing a periodontist & best of the best

It was my first ever time seeing a periodontist, coming in with a bunch of terrible gum, teeth, and jaw bone infections, and I feel like I won a trillion dollar lottery by choosing Dr. Elly Tehrani as my doctor. She is simply the best of the best, out of this world good. And her team is also great, all of them. Highly recommended.
