i read some dr used prp in fat transfer ( The fat that has PRP added is called “SmartFat”. SmartFat can be thought of as sending “text messages” to the body to help promote fat cell survival. PRP also helps the stem cells within the SmartFat to excrete a hormone that helps attract blood to an area) to make the risul preminant ?
I have attached a link to the ASPS position paper on Stem Cells in Plastic Surgery. People like something 'new' and marketers will cater to that weakness. There is no scientific evidence to support these claims.
It seems a fat graft implant is best to fill the void left from an over-resection of glandular tissue from beneath my areolas when I was just 15 yrs old. I am now 43 yrs old and I am hoping the fat graft implant is a permanent solution, even if it requires two procedures (given the nature of fat survival). My question: Will the fat graft provide a similar firmness and shape that glandular tissue normally provides--enabling the areolas to protrude slightly even w/ arms raised?
Fat grafting is probably the only reasonable solution for this problem. It may take more than one session, but you seem to be prepared for that.

Doctors say they extract stem cells from some of the unused fat, and then add that to the fat to be injected, multiplying the stem cell density. Seems like it would be hard to have enough extra fat to do this for large areas but maybe for the face? Does this stem cell extraction/concentration work? Is there another technique besides a centrifuge?
It is very unlikely to be of any benefit at this time. I have attached a position paper from the ASPS that makes the point that there is no scientific basis for stem cell use is aesthetic surgery.
There are Board Certified Surgeons that are using these claims too.
