Can my Breasts Be Lifted Without Implants? (photo)

I've nursed three children and had a 70 lb. weight loss that have left me with lots of excess skin & sagging. Once I reach my goal weight, I'm interested in a mommy makeover that includes a tummy tuck and breast lift, but not augmenting my breasts. Do you think that's possible with how my breasts look? I'm happy with the size of my breasts when I wear a good bra (currently 38DD) but unhappy with how deflated & saggy they are without a bra.

A: breast lift without implants is possible in some cases

Based on your photographs, you probably do have enough breast tissue to have a mastopexy (breast lift) alone without implants. This would, in my opinion, require an inverted-T incision pattern. Without implants, your breast size will decrease even if no breast tissue is removed. There will also be little change in the upper pole fullness of the breast (roundness on the top) without implants. You should nonetheless be able to get a nice result, based on the photos. Of course, a thorough exam and consultation is needed to clarify your goals and the best options for you. Thanks for your question.

What Can an Athlete Do About Training when Recovering from a Breast Lift WithOUT Implants?

I'm a 36 year old athlete getting a breast lift WITHOUT implants. I haven't yet had my consultations, but I know that I'll be receiving a lollipop or anchor procedure, and I a worry about the impact of recovery on my training. I lift weights intensely 3x a week and do plyometrics and interval training the other days. What is the timeline reality of being able to resume this regimen after surgery? Are there plans for athletes? I worry about losing endurance, muscle tone, and strength.

A: No restrictions 2 weeks after breast lift

In general, I will let you work out without restrictions 2 weeks after mastopexy. Prior to this, there is a low, but real, risk of  bleeding. You can swim as soon as the incisions are no longer crusting, usually about 3 weeks postop. You can walk and do light activity as soon as you feel safe to do so after surgery. If you work out regularly, you should be back to a normal regimen quickly.

Does a Lift Without Implants Also Reshape the Tissue,or Just Alter Skin and the Nipple?

I am a C cup, and plan to get a lift with no implant. I have significant sagging, and will surely be having a vertical or anchor procedure (consultations are in September). With either of those incisions, is it typical for a p.s. to also internally provide more support for the breast tissue, itself, or is the lift simply a skin and nipple relocation? Is there some procedure I should be asking about besides the type of incision which will make my breasts "sturdier" beyond the skin tightening?

A: Breast implants will give fullness to the upper breast when used with a lift

In general, if you have significant skin excess, an upside-down-T/inverted-T mastopexy (breast lift) is required. If you have enough breast tissue, you will often have good breast fullness in the mid and lower portions of the breast, although the breast size and bra size will decrease just from the lift. A breast implant is useful (1) to maintain or increase size, (2) to increase the "upper pole" fullness (fullness in the upper aspect of the breast); (3) to improve the roundness and projection of the breast. In general, if the upper breast is flat before a lift, it will be flat after the lift settles.

I personally do use internal sutures to improve the roundness and overall shape of the breast, as do most of the other surgeons who have answered your question. However this is dependent on having enough breast tissue to create a nice shape. Your surgeon should be able to explain differences between the using or not using an implant.
