I had thermage done when I was 23 and I went from being full faced to looking quite gaunt. As a result, I think it took a toll the structured support under my eyes. If I get an injectable filler, such as restylane, could this help my tired-looking under eyes? Or should I just get a really good under eye cream? I'm only 25. Thanks!
Several things happen under the eyes as we age. Some folks are unlucky enough to have these changes show even in the mid-20's but it is commonly seen in most people by the mid-30's. Crepey or wrinkly changes under the lids are responsive to medicated skin products like retinoids and others as well as to chemical or laser peels. Deepning of the tear trough accents the transition from eyelid skin above to cheek skin below. This is commonly treated with injectable fillers such as restylane. Bulging fat in the lower lids requires lower eyelidplasty altough early bulging can be temporarily camouflaged with fillers to some extent.
I had two syringes of Restylane four days ago, why do I see no results? The injecter suggested that I have one syringe in the upper check area on both sides hoping to lift up and out because I have lost weight in my face. I shared that I wanted my checks to look fuller and I was concerned about the lines on both side of my mouth. I could hear and feel the injection into my upper check bone. I was bruised from the inside but with ice no sign of any bruising on the outside. What do I do?
Unfortunately, it is not totally uncommon to feel like you invested so much time and discomfort, and got not much out of it. The treatments when done by a well-qualified, safe physician injector are not cheap (you get what you pay for to some degree... so this is valid), and it can be surprising to patients to spend several hundred dollars and not feel "totally fixed". This has to do with expectations being set by the physician, and expectations brought to the treatment by the patient. It is unfortunately true that everyone has different facial structure, tissue density, and volume loss that needs correction. Patients also have different expectations of what they are hoping for when they come in. Some people truly do need two, three, or even rarely four full syringes of filler. This is just fact. Patients who can't afford to or don't want to spend a lot, may negotiate with the doctor to do only one syringe... and then end up disappointed or feel like "nothing changed". It's important to have a realistic conversation before you start, and then to remember that conversation after the initial swelling has reduced and it may be time for a little additional filler. Yes, expensive. Yes, likely worth it, with the right doctor.
Has the size of the needle changed over the years to improve the injection process of fillers like Restylane, or is it more in the technique?
Generally, the needle size of the fillers has stayed the same (smaller needle for the thinner fillers, bigger needle for the thicker ones), but there has been some advancement in the details of the needle engineering and design that are intended to handle the fillers for a smoother delivery and more reliable performance. However, of course with more experience over years of use, techniques have also advanced over the years. The result is that fully educated, well trained physician injectors in the four core facial cosmetic specialties (board certified dermatologists, plastic surgeons, oculoplastic surgeons, and ENT/facial plastic surgeons) are getting better and better at achieving great results with fewer side effects.
