24 Yr Old White Female with Red Telangiectases Veins on Chest- Options? (photo)

I am a 24 yr old female with red Telangiectases superficial spider veins on my chest (a few on bridge of nose). I tanned regularly in university, burning a few times. I was on 100mg spironolactone for 2 mths, then went on accutane (20mg/day 120 lb) 5 mths (this worked for my acne), but the veins seem to be getting worse daily. I am very concerned given I am only 24. I am considering laser in a few months but want to understand the cause. I feel ugly, any suggestions or advice would be great.

A: Treatment Options for Chest Sun Damage

Based on the photos which are somewhat blurry, it appears that you have a combination of redness (mostly related to the broken blood vessels) and brown discoloration.  These are both likely secondary to sun damage, and are more common in fair-skinned individuals.  A series of intense pulse light (IPL) treatments can help improve this with minimal downtime.  Also very effective but requiring more healing would be a combination of the erbium laser to the brown spots and light electrodessication (superficial burning) to the broken blood vessels. The most important thing you can do at this point is to protect the areas from future sun damage by practicing sun safe habits.

A: Painful Vein Under the Breast

There are several reasons why the vein under your breast may be painful.  It may be due to vein thrombosis, which in tern causes inflammation and pain, it may also be due something underlying the vein and has nothing to do with the vein itself.  The best thing to do is to see a Breast Surgeon right away and ensure that there is nothing more significant going on. 

Sometimes non-steroidal anti-inflammatory medications can help, but before treating yourself, make sure you see a physician.

Laser on Upper Lip for Fine Red Vein, Is it Possible to Receive Nerve Trauma?

following on I am worried that my surgeon may have had the setting too high when he did the treatment on my red vein on my upper lip as it felt too powerful. He then treated the sides of my nose which were fine in comparison. If he did have the setting too high for my upper lip, and the fact i have an inplant in my upper lip, could the nerves have been too close to the suffice of my skin and therefore could i have received nerve trauma which then resulted in trigenial neuralgia?

A: Nerve damage with laser treatment

It is extremely unlikely that you may have sustained any type of nerve damage with laser treatment for broken superficial capillaries.  The reason you upper lip felt more sensitive during the treatment is because the skin is typically thinner in that area and therefore treatment tends to be more uncomfortable.  As long as you do not have any residual burn marks or wounds post treatment, any discomfort you had during or right post treatment will resolve.
