Cheek Filler or Mid Face Lift?

I am 31 years old. I have extremely hollow flat cheeks, hollow eyes, and now pre jowls and lots of new lines forming and dents on chin. My skin is very much lacking elasticity as it feels loose and no firmness. All of this happenend within 1 year (ever since I turned 30). I believe I have prematurely aged (8 to 10) years sooner than normal as I have been a smoker. I am extremely depressed about these changes. I used to have a heart shape face and now it looks long & flat, pre jowls. Help?

A: Cheek Filler, Fat Injections or Midface Lift

Fillers like restylane, juvederm, radiesse or perlane can be very effective in restoring midfacial volume in the cheeks and restoring youthful fullness in a young person who feels they have prematurely aged.   Hyaluronic acid fillers like restylane or juvederm are currently the only safe fillers in the periorbital region but must be done by an experienced injector.  Artful injection is required to create a natural looking eyelid/cheek junction. Alternatively, microfat transfer can provide more lasting improvement that is softer but requires more downtime than fillers.   I think either would be appropriate for you from what I can tell from your picture.  I usually reserve an endoscopic midface lift for patients with fuller cheeks that show evidence of drooping like hollow eyes, a deep nasolabial fold or buccal fat herniation.  An endoscopic midface lift with fat repositioning of the orbital fat, buccal fat, and malar fat pads can be very effective in these situations to restore facial volume and lift a sagging face.

Cheek Lift and Other Options for Patients in Their 20s

I am 23. I have droopy cheeks, extra skin/fat in the jowl/chin area, and visible lower eye whites. When I smile these issues are accentuated. Would a cheek lift address these issues and how do I find facial surgeons who have extensive experience on younger patients?

A: Cheek lift or Midface Lift and other options

I have a lot of experience with cheek lifts or midface lifts in younger people and my preferred technique is the endoscopic forehead/midface lift with buccal fat transposition.  Midfacial aging does begin to occur in the mid to late 20s.  But generally mid to late 30s  is the minimum age I would consider for this procedure.  Sounds like your issues may be somewhat congenital or skeletal since you have them at such a young age.  Adding volume whether with microfat transfer, fillers, sculptra or cheek implants is probably the way to go at your age but difficult to tell without a thorough evaluation.   Adding volume with one of these methods can give you the same look as having a lift with less trouble.

Will Cheek Lift Improve Hollow Lower Eyelids ?

I had an aggressive lower blepharoplasty with excessive fat removal. I am now considering a Lower face lift/Neck lift and the surgeon wants to raise the cheeks as part of the lift. I understand this will give me more volume up to the orbital rim, but my hollowness is above the rim.

Given the extra healing time, would I not be better skipping the Cheek lift and treating the lower eyelid with fillers or fat? Thanks

A: Cheek lift never addresses the medial orbital rim

Some cheek lifts will improve the outer half of the orbital rim, but not the innner half. It will also not correct a hollow above the rim.

Fat injection - Viafill is best - will correct these problems. Safe simple and avoids surgery.
