Pre-Op Worry About Pain And Problems Associated With Surgery

I am scheduled for a breast lift with silicone implants 350 under the muscle. I am 53. I am suddenly very nervous as I have read reports of the pain and sproblems associated with this surgery. Am I getting worked up for nothing? Thank-you so much.

A: Preop anxiety about pain and complications It is completely appropriate for you to be concerned about pain and complications. Frankly, when patients don't worry and slough off these concerns, I get worried!! An informed patient is a patient who will participate well in her own care. No surgery is pain-free. However, what you should know is that the pain from augmentation under the muscle is like a faucet--it turns off within 72 hours when the muscle stops contracting. Your plastic surgeon should be able to provide you with reasonable prescription pain medication to get you through this period. Definitely discuss pain treatment alternatives prior to the procedure as this should put your mind at ease.

Do I Need a Breast Lift During Implant Replacement?

I have Saline breast implants that are now ten years old. They were beautifully done and look natural, although the doctor went bigger than I had asked for: a full C instead of a full B. It was his aesthetic call, he said, and at the time, (and urged on by my then-boyfriend) I decided I'd live with them.

Ten years later, I am about ten pounds less and much leaner than back then (5'5", 112 lbs, very fit, 18.5% body fat) and the implants have sagged somewhat. When I see photos of myself, I feel my silhouette is out of proportion, and that I would look much better with slightly smaller breasts, as I had originally intended... and perhaps Silicone this time. To go for smaller, firmer, higher breasts after having larger saline implants, would I need a Breast Lift in addition in order to accomplish this? How are these procedures usually done?

A: Exchange of implants with mastopexy You are correct. If you replace your implants with smaller ones, after 10 years at your current size, you can expect that the breasts would sag so it would be reasonable to combine the reaugment with a lift. There are many decisions to make in planning the combined surgery (silicone v. saline, above v. below the muscle, periareolar v. T-incision, etc.) Definitely see a board certified plastic surgeon to begin to plan for this surgery. If you had infra-mammary scars from the breast augment, these could be used in the mastopexy. As women with breast augments age, I am seeing a significant number of patients who desire this procedure.

How Long Does a Breast Lift Last?

long wanted to get a breast lift - mastopexy - but worried I'll spend thousands and end up with the same saggy breasts i started with. Am i right to be concerned?

A: Duration of a breast lift There are several important considerations in breast lift. First is undergoing the appropriate procedure for the size and shape of your breasts. The donut or Benelli mastopexy will not last as long as the full mastopexy which has more visible scars. Secondly, if you combine a lift with augmentation, know that the weight of the implant may ultimately cause the breasts to sag. Generally, the smaller the breast, the less risk of recurrence. There is a balance to be drawn between shape/contour and scarring. If you are considering this operation, it is also critical for you to maintain a consistent weight as yo-yo dieting can also affect the result.
