Candidate For Transconjunctival Blepharoplasty or Restylane Filler?

I am 27 years old and suffer from under-eye puffiness. I'm in dire need to correct this situation in order to restore my youth and happiness and not hide behind my glasses as I've been doing for years. Am I candidate for the less evasive Restylane filler? Or would you recommend full Transconjunctival Blepharoplasty?

A: Transconjunctival blepharoplasty or Restylane filler for lower eyelid bags

As you can see from the various answers there is certainly a difference in opinion on how to handle your situation. I have found that in cases of young individuals like yourself with obvious herniated lower eyelid fat, that a transconjunctival blepharoplasty can achieve very satisfying results. This does not address the fact that your mid face is not very prominent. That would be better addressed with an injectable filler but I think that this would not be a significant issue if you no longer had to deal with the bags.

Surgically we have become more conservative with fat removal over recent years but in your case, I believe that straight forward removal, if judicially performed will give you a nice result without any sort of "sunken-in" appearance.

Transconjunctival vs. Transcutaneous Blepharoplasty

I am a 34 year old male with eye bags. I have gotten two opinions on how to treat them. Now I am confused. Is one procedure safer than the other? How long do the results last?

A: Transconjunctival vs. Transcutaneous Blepharoplasty - which is better?

Based on your photos, as a young individual whose predominant problem is puffiness to the lower lids, you are an ideal candidate for a transconjunctival blepharoplasty. The minimal amount of excess skin you have  will re-drape nicely after the fat is removed, and if performed by an experienced surgeon, the healing time with the transconjunctival approach will be very short with minimal discomfort. Both approaches are quite safe when applied appropriately since the chance of ectropion in someone your age is minimal. But without any excess skin there is really no reason to use the transcutaneous approach.

Leech Therapy for Blood Pooling - Is This Possible?

I had upper and lower eyelid surgery, I have a pool of blood (lump) under one eye. I was told about Leech therapy? from a friend it sounds so horrible any info on this?

A: Leech therapy for small blood lump after eyelid surgery? NO.

The modern use of leeches really began in the early 1980’s with the development of free flaps and large myocutaneous flaps for reconstructive purposes if the venous drainage to the flaps was initially inadequate, they would ultimately fail due to congestion. The leeches, by removing the congested blood during that initial period, allowed many flaps to ultimately survive. That said, they certainly do not have any place in cosmetic surgery. If you have a small area of pooled bleed and it is about one week post-operatively, then the pool of blood can be evacuated as it begins to soften with either a needle or a small stab incision. Or you can just allow it to resolve on its own.
