Natural Looking Breast Implants with Transaxillary Incision?

I see many photos of underarm incision for breast implants, and the implant placement looks oddly high and often misplaced. Also, many surgeons won't use this incision.

Is it possible to get a natural look with the underarm incision for silicone breast implant? I am a first time patient, size A, looking to increase to full B.

 

A: Consider endoscopic transaxillary approach

There are many approaches used for breast augmentation. The trans-axillary approach is especially appealing since it avoids incisions in and around the breast.

However, the only way to safely and precisely perform a breast augmentation through the axilla is to use an endoscope. In this way the pocket can be clearly visualized and proper implant placement can be achieved.

Many doctors are not comfortable with the use of the endoscope. They either do not perform this procedure or use blind, blunt dissection to create the pocket. This can result in asymmetry and high placement of the implant,

The pictures you have seen likely represent poor pocket dissection using a blind approach.

Keep in mind that generally the largest silicone implant that can be placed through the armpit is about 250cc. If you desire a larger implant, then you may have to consider a saline implant.

Should I Switch to High Profile Breast Implants?

I have 420cc low profile in right now. My frame is small and I feel like I have a pancake inside. I have never felt the implant projected as much as I would like. (I still have to wear a padded bra) I also feel like they are too wide for my frame.

I would like to go for a high profile 425cc or 475cc. Do you think the projection I would get with one of those compared to what I have now would be enough for me to see the difference through my clothes?

A: Selecting the proper implant

The choice of implant often depends on the patient's anatomy.

Generally speaking, higher profile implants have a narrower diameter and would work better in a patient with a smaller frame. Furthermore, high profile implant project forward more, so they will appear bigger than a low profile implant of the same volume.

Another factor to consider is that high profile implants tend to cause less rippling. This is especially important in thin individuals.

Based on your description, you have a small frame and may benefit form high profile implants.

Keep in mind that a a 425cc high profile will seem a lot bigger than a 420cc low profile implant because the high profile implant sticks out more.

Depending on your anatomy and the type of clothes you wear, the high profile implants should give you a significant noticeable difference through clothes.

When Should I Have Breast Implants Replaced?

I had a breast augmentation in December 2000. I was wondering, when do I need to have them replaced? Any feedback would be helpful. Thanks!

A: Breast implants don't last forever

Dear Emily,

There are 2 components to all implants: The shell and the filler material.

The shell in all FDA approved implants available in the USA are silicone. The filler material can be either saline or silicone.

The saline is basically salt water at the concentration that is equal to our body's salt water concentration. So if it leaks, it gets absorbed by the body with no harmful effects. What the patient would notice is that the breast that has the leaking implant has decreased in size.

The silicone is a gel made of a material that is kinda like jello, but it is very sticky. The silicone gel material has changed over the years. The newer material is called the gummy bear silicone. This is because it is less likely to travel if it leaks.

Keep in mind that nothing lasts forever. All materials degrade over time. This is also true of breast implants.

Studies have shown that the leak rate of breast implants increases significantly after 10 years.

If you had your augmentation in 2000, you most likely have saline implants.

Saline implants generally do not have to be changed unless you are having a problem with them.

In the case of silicone, you need to be followed with regularly scheduled MRIs to make sure there is no leakage.

Furthermore, we currently recommend implant replacement in patients with silicone implants that are more than 10 years old.
