I Need Some Advice About 500 Cc's Saline Implant Placement. Above or Below Muscle?

I've had 2 consults with 2 diff PSs & they both had diff suggestions.The first PS suggested Cresent Lift,500cc Saline Anatomical Implant placed over the muscle.The 2nd PS was completely against a cresent & an anatomical implant & suggested Benelli Lift, 500cc Saline implant placed under the muscle.I've done so much reading & research and I'm just left feeling more confused than before! Initially I thought a 500cc Saline implant under the muscle with a Cresent Lift would be my answer...HELP! :)

Without photos, it is impossible to advise you on which form of breast lift would be best for you and your unique anatomy.  

If you want to start a great discussion in a group of plastic surgeons, just ask which is better, round or anatomic.  There really is no right answer and it really comes down to which implant the surgeon feels most comfortable with and they believe will give them the best result.

Lastly, saline implants above the muscle do have increased risks for rippling and puckering compared to saline implants under the muscle.  While this may not be a problem initially, as you mature and your breast tissue thins in the upper pole of the breast, this can be a problem in the future.  Certainly something I would recommend you discuss with your plastic surgeon.

3 Months Postop 500cc Implants with Internal Lift. Is It Normal They're Still Firm?

im 3 months post op breast implants size 350 overfilled to 500cc. i like the size but they are still firm. will they soften up and feel more natural in over the next few months?

A: Overfilled Saline Breast Implants and Firmness

Dear Blessed in Ga,

At three months out from your breast augmentation, there are two common sources of persistent breast firmness.  One is the possibility of a capsular contracture which is a tightening of the wall your body makes around the breast implant.  This is something your plastic surgeon can determine with an examination.  The second is the significant degree of overfilling of your breast implants.  Saline implants are frequently overfilled by surgeons and the manufactures give a range for optimal fill.  350cc implants that have been filled to 500cc in volume are beyond the usual recommendations and can be the source of your persistent breast firmness.  The fact that both of your breasts are firm increases the probability that the source of your breast firmness is the overfilling of your implants and not a contracture.

Please follow up with your plastic surgeon who can discuss and advise you best.

MRI Indicates Bilateral Rupture of my Silicone Implants, But Dr. Wants More Results. Why?

I was in an car accident 5 months ago. My front airbag deployed and my car was totaled when someone pulled out in front of me while I was going 50 MPH. I have had pain n both ever since. My surgeon ordered an MRI that indicated a biltateral rupture. He doesn't believe and wants me to get a 3D MRI. With all the cost and the fact that I have so much pain and my left implant is softer I dont' know why he doesn't believe the MRI results? He says to massage the implants and the pain should go away.

A: MRI Results and Ruptured Breast Implants

Dear Deadsexee,

While MRIs are the most accurate imaging test currently available for ruling out implant ruptures in silicone breast implants, they are not perfect and can give a "false-positive" result at times.  Almost every plastic surgeon has had the experience of having a MRI result that is positive for an implant rupture and then finds the implant intact in the operating room.  Again, no test is 100% accurate.

Because of your concerns, please discuss your worries with your plastic surgeon including the issues of the extra costs for the additional MRI.  Other imaging options can include a high-definition, 3-D ultrasound study which has been shown in studies to be very helpful in determining is there is an implant rupture in silicone implants.  If you are still unhappy with your plastic surgeon's response, please consider a second opinion but be sure to have your MRI films or disc and results with you when you go.

I know this is a difficult and emotion time for you after you accident.  It is important to confirm your implant rupture to rule out unnecessary surgery.  Best of luck.
