Fraxel Repair Vs Dermabrasion to Complement Neck Lift?

One Dr. says Fraxel is it! Other says Fraxel Repair benefits don't last after swelling goes down. 57 yr old with lines around the mouth some sun damage, moderate wrinkling. No eye problems except some crow's feet.

I will be having Neck lift with either Fraxel or Dermabrasion. Would like to hear from Drs and also other people who have experienced one procedure or the other.

A: Fraxel repair

I commonly perform Fraxel repair treatments with minilifts, Necklifts and the two combined. Dermabrasion is largely out of favor these days. Fraxel repair will improve mouth lines but will not completely eliminate them, particularly the deeper ones.

It also has a lower incidence of complications than dermabrasion or fully ablative CO2 Laser resurfacing. It is also a painless recovery.

The benefits of fraxel repair persist for at least a year, certainly well after the swelling resolved, as has been documented in many histologic studies performed during the FDA approval process.

Is PIH Following Fraxel Repair Permanent?

I am being treated with 4% hydroquinone for the past 4 months for post-inflammatory hyperpigmentation (PIH) following Fraxel Repair. The hydroquinone does not seem to be working. Is PIH permanent? If not, will it eventually disappear on its own? Furthermore, if it ever gets resolved, is there a chance that it may come back?

A: PIH is not permanent

PIH after repair occurs most commonly in people with more pigmented skin. If 4% hydroquinone does not work you may try a combination treatment such as TriLuma which has HQ, Retin-A and a steroid. The combination works well to eliminate PIH.

Total FX or Fraxel Repair Done As a Series of 3 or 4 Treatments Rather Than One?

If Total FX or Fraxel Repair were done 3 times or more for various aging issues (on face, neck and chest) over a period of months, would the results be cummulative, or can scarring develop if its done too many times? This is all assuming (obviously) that the treatment is tolerated well the first time.

My MD, (who works with burn victims at the VA hospital in Palo Alto and is an associate professor at Stanford and has a ton of experience with full Co2 and Total FX) states that if done conservatively, it can be done more than one time if desired. Any comments?

A: Do one treatment and wait at least six months

I have performed repeat Fraxel repair treatments but only after waiting approximately six months. The two primary reasons to wait include letting the skin heal and knowing that the collagen formation from the treatments continues for at least six months. So you don't really have a sense of the results from the treatment until about six months later. That being said, the results after the second treatments really look fantastic, perhaps because of the hybrid protocol we have developed over the past year.

Best,
