I had too much taken out on one side of my outer thigh(not enough in one spot on the other) but more to the back of my thigh. The shin looks healthy,I do not think it was so deep as to cause capillary damage but enough to consider fat transfer back into the area. I am 46 5'9' and weigh usually between 127-132,I have had a very healthy lifestyle for most of my life,mostly raw foods and am quite athletic. Would a micro liposuction combined with a fat graft(properly) grafted help my situation?
Without reviewing your pictures, it is difficult to provide you with accurate assessment and recommendation; however, autologous fat grafting is a mainstay of correction of such defects. In my practice, I also utilize fat redistribution after VASER emulsification of the defect area. This allows for the fat in the area of the defect to be redistributed to correct the defect without harvesting and transplantation which reduces survival especially in a previously treated area with significant fibrosis. I would advise seeking consultation with a surgeon experienced in fat transfers and corrections of lipoplasty deformity.
Will fat grafting work to fill my tear troughs? Also what to do about loose skin under the eye?
I have used autologous fat injection very successfully for midface volume enhancement including the tear trough. This may lead to permenant improvement with good fat harvesting and transfer techniques. It is important to find a surgeon who is not only experienced with tear trough injection but also with autologous fat transfer techniques. To see how it may look, you may try a hyaluronic acid filler ( I like Juvederm) which can be dissolved if its not what you expected unlike fat which may be permenant. I frequently recommend cheek augmentation along with tear trough injection.
Undereye fat transfer: Is it better to undergo local liposuction or moderate kenalog injection to reduce unevenness in a fat transfer procedure?
It depends on how long ago you had the injection. In the initial 2-3 weeks it is not uncommon to have some asymetry but if its been months and you have a noticible difference under your eyes, there are several options. You can have additional soft tissue injected to the deficient side to achieve better symmetry. You can also try to reduce or remove the excess fat by steroid and/or 5-FU injections. Steroids can also cause subdermal lipoatrophy that can create skin dimpling. You can also have a localized subdermal laser lipolysis treatment with Smartlipo or similar commercial LASER. Lastly, you can have it surgically removed by localized liposuction or fat excision going through a blepheroplasty incision or gingivobuccal approach (through the junction of cheeck and gums). My preferance would be to start from the most conservative and least invasive option.
