I'm 23 Years Old and my Face Sags. Would a Mid-Facelift Help?

I'm 23 years old and my face sags especially around the mouth. I'm not over weight, I stay out of the sun and live a healthy life style so I'm not sure why my face is like this. I've been to a dermatologist and while they haven't ran any tests they say "its just genetics". I'm curious if It looks like I would benefit from a lift and/or lipo and fat grafting or somthing? Thank you in advance!

A: Young Patient Concerned with Sagging Skin

Hi malaka ,

20's is generally too young for facelift surgery, including midface lift. Other aesthetic options to improve one's appearance at this age include such treatments as skin care, laser, light peels, or tretinoin.

Based on the photographs, your jaw bone may be relatively small and unbalanced with the face. This small chin or jaw may contributing to the "sagging around the mouth" that concerns you. Jaw surgery, with or without chin implant, may possibly improve the appearance. In addition, fat injections or injectable fillers may also improve the area.

Only after a comprehensive evaluation by a plastic surgeon can he/she help determine appropriate options for you. Best of luck.

Cheek Lift at Same Time As Fat Transfer?

I am planning on having fat grafting in a few months. And my ultimate goal is for the best fat retention/survival possible. But I am toying with the idea of having an endoscopic cheek life. Would it be ok to have this done at the same time? Or would it be better to wait. Would having additional surgery at the time of the fat transfer compromise the potential fat survival?

Midface or cheek lifts are plastic surgery procedures to improve the middle portion of the face & cheeks. Cheek lift plastic surgery may be performed via multiple approaches. This facial area is also commonly treated with fat injections as part of facial rejuvenation. 

Depending on the specific cosmetic needs of each patient, fat injections may be combined at the same time of a cheek lift. Each procedure address different components of facial aging. Fat graft survival after injection is variable to a degree, as some fat will always fade with time. Touch-up procedures may be required to maintain results. After a comprehensive evaluation can a plastic surgeon help determine appropriate options for you. Best of luck.

Can Cheek Lift Pull the Lower Eyelid Down?

I read on some of your post where a cheeklift could be use to tighten lower lids. Then I read it could pull the lower lid down. I am wanting to get upper and lower lids done. I want my lower lids tighten. I have the empty space between eyes and cheek area. I have wrinkles under eyes and extra skin and fat . I am 41 years old. Does a cheeklift help or pull the lower lid down? I need a facelift but am to scare for that just yet. Thanks.

A: Cheeklift and the Lower Eyelids

Everything that you have said is correct and the surgeon with whom you consult will tell you that all of the complications you have mentioned can occur with a cheek lift. From the looks of your photo, it appears that you are deficient in the midface and have some bagging (psuedofat herniation) of the lower lids. Your upper lids look decent from the photo you have provided. From the photo, however, you may want to consider interventions that augment your midface area. These can be fillers, fat, and even cheek/midface implants (not my first choice but it works well for some patients with thicker skin). Other surgical interventions would be a mid-face lift in conjunction with lower eyelid surgery. This would help with the bags and hollows beneath your eyes but, can lead to lower lid ectropion and you would still require some soft tissue augmentation as you are quite thin and need some fullness to the midface area.
