I have some acne scars on my face, most are ice pick and rolling scars. Now I know there are 3 companies that provide laser treatment: Fraxel restore/repair, Palomar fractional laser, and Deep FX and Total FX. Which among the treatments is best for me?
Acne scars can be diffficult to treat even in the most expert hands. Fractional resurfacing typically results in improvement, however improvement can range widely from 20 - 90%. A recently released XD optic for the Palomar Starlux and Palomar Icon systems was designed to deliver energy more deeply. We have seen responses on ice pick and other acne scars with this new device that have not responded to other non-ablative fractional lasers.
Are the results with the new Fraxel Re:pair progressive as they are with the Re:Store?
I heard that results with the Fraxel are somewhat immediate for acne scars, but they get a lot better as the months go by after treatments. Is this true with the new Re:Pair too? I would only think that being it goes so much deeper that it takes time for the deeper wounds to heal and produces collagen. Is this true?

Yes, in fact after a Fraxel repair treatment session, the collagen remodelling process continues so that further improvement is often seen for about 6 months.
Am I a candidate for Fraxel re:pair if I've already had silicone microdroplets injected into acne scars? My intent is to diminish the same scars (not wrinkles), so I need to be sure that I could use the highest possible settings to make the procedure worthwhile.
Unfortunately, there is no clinical data to support the safety of Fraxel repair after silicone injections for scars. There have been some reports of inflammation at the site of silicone injections after subsequent laser treatments. This is an area where there just is not enough information to be able to give the kind of advice that as a dermatologist and laser surgeon that I could recommend.
