I have a Indian Skin, I have Deep Pitted Acne Scars. I do not have Fraxel Repair available in my city or in my country If I wish to go for the Fraxel Restore and If I start with a Higher Preset from my first session i.e 70mj are there any chances of Hyper-Pigmentation on my skin because of using a higher preset. Plus I have a very sensitive skin. But without having a Higher Preset my scars will not respond as they are that deep.
If Fraxel setting are aggressive (as you suggested at 70mJ), the risk of hyperpigmentation is very real. I usually use lower fluences and lesser densities (area of coverage) to prevent this complication. For that reason, Fraxel Restore in my practice is done with less aggresssive, but more frequent treatments. I also usually pre-treat with retinoids and hydroquinone, make sure sunscreen is worn daily, and I try to do the procedures in the less sunny winter months.
Is it dangerous to Fraxel treatments yearly? Is it common to have certain area of the body treated more often than others? For example, would the face and decolletage upper chest area need treatment more regularly?
No matter what type of Fraxel treatment you get (ablative or nonablative), it is safe and smart to get yearly treatments. Your skin will, unfortunately, continue to age, and re-treating will help you regain lost collagen and help smooth out wrinkles that are re-forming. Areas with more photodamage will probably show signs of aging more quickly, so they may need more frequent treatments. This is something you should follow and discuss with your Dermatologist or Plastic Surgeon. Remember to protect your skin from ongoing sundamage with sunscreen and antioxidants, and to continue the repair process with topical retinoids and growth factors. A good skincare maintenance regimen will help you to continue the good results from Fraxel treatment.
Is it better to do BHA peel after a series of Fraxel..
Depressed scars will get much more benefit from Fraxel treatment, since the Fraxel laser penetrates so much more deeply than a BHA peel. BHA peels will help to treat ongoing acne, pigmentary problems like brown spots, and mild (not deep) textural issues. So, if your focus is the scar, do the Fraxel first. if you also have problems with acne or pigment, you can do the BHA peel before or in between Fraxel.
