Will Scar from Subcutaneous Forehead Scar Be Noticeable?

I am going to undergo a subcutaneous forehead lift as I do not want my hairline to go backwards. Will the scar be noticeable or not?

A: Pre-hairline Incisions Can Heal Very Nicely...

Pre-ahirline incisions can heal very nicely if done properly. You should make sure your surgeon has a lot of experience with the "trichophytic technique" which insures that hair will actually grow in front of the incision. This is the key. Additionally we rarely extend our incisions into the temporal area although others still do. In our opinion little is gained from this extension and there is a possibility that thinning of the hair in front of the incision can occur!

Coronal Forehead Lift for Young Patient?

I am in mid 20's (female) but already have issues with horizontal forehead wrinkles/folds, very apparent even with the slightest effort to look up.

Today, I met with my plastic surgeon (who performed Asian blepharoplasty on me few years ago with very satisfactory/natural results), and he recommended Forehead lift for lasting results.

I did some research later and found out what he explained to me was "coronal Forehead lift," which seemed unnecessarily invasive especially for a patient in her 20's. I called his office, and apparently he does not perform Endoscopic Forehead lift. Is that normal, should I find another doctor?

A: Forehead Lifting Approaches

In my opinion anyone doing forehead lifting procedures should be skilled in all of the available techniques to accomplish elevation of the brows. Each technique has it's own unique indications. These include the coronal approach, the pre-hairline (trichophytic) approach, the trans-blepharoplasty approach, the mid-forehead approach, and of course the endoscopic approach. In our practice we usually do 50% of our lifts via the endoscopic approach and the remainder as a combination of the other approaches mentioned above. We rarely, however, utilize the coronal approach given the other alternatives.

Will Forehead Tucks Help Make the Eye Area Less Droopy?

Someone suggested "forehead tucks" on each side of the forehead to make the area under my eyebrows and over my outer eyelids look less droopy. I had a blepharoplasty in 2003, although I was not happy with the results because there's too much scarring and my eyes are uneven.

Do you surgeons feel that the bilateral forehead tucks would improve my appearance? I have permanent eyebrows tattooed on, but will get them lasered off.

A: Endoscopic Browlift is the procedure of choice

While I cannot see your hairline in the picture it appears that a conservative endoscopic browlift would be very helpful. If your hairline is already elevated then a modified endoscopic browlift using a trichophytic "pre-hairline" incision would be indicated. As far as laser removal of the tatoo, be very careful, since you can potentially lose any remaining hairs in your eyebrow.
