Silicone or Saline Implants?

I'm a 48 y/o woman, 5'3 150lbs...hoping to lose another 25lbs. I am scheduled for a mommy make-over and am still indecisive about whether to go with Silicone or Saline and considering 450-500cc implants. I am aware of the pros and cons for each, however, I'm wondering which one's would give my breasts the best results. My doctor is going under the muscle for the procedure. Any advice/opinions will be greatly appreciated.

A: Saline/silicone - you will be happy with either, but do you need a lift?

Saline versus silicone?  Keep in mind you will be happy with either and when the implant is under the muscle and you have some of your own breast tissue (you do), the difference in feel characteristics is small.  Usually I use saline implants in this situation simply because there is negligible advantage in using silicone gel and it is more expensive.

The more important question is whether you need a lift.  You have significant asymmetry and your nipple level is particularly low on the left side.  An augmentation does not correct this degree of sagging and does nothing to improve nipple symmetry.  If you have a simultaneous lift, it should be a vertical lift, which offers better shape and less scarring.  If you just want the volume and don't care too much about nipple position or symmetry, just have the implants.  If you wish to look at some photos of lifts+ implants, I have a link to this section of my website, so you can get an idea of the results.

Will a Warranty Cover The Implant and Partial Costs if Breast Implants are Leaking?

I had my aug 6/99. Saline,but I'm not sure of the brand.About 3 years ago,my right breast seemed smaller so I went to my surgeon to discuss if it were possibly leaking.He said it was not.It kept getting smaller, but not terribly noticeable.I went back to him before my 10 year anniversary to again inquire it it may be leaking and again he said it was not.Over the past few days,my right breast has gotten about a cup size smaller.Wondering if the warranty will cover the implant and partial costs?

A: You have a leaky implant, needs to be replaced, outside warranty.

Sorry, but the warranty has expired.  The standard warranty in 1999 was for 5 years, unless you purchased additional protection, so it expired in 2004. 

If your breast has dropped this much smaller, almost certainly you have a leak.  So you will need to have your implants replaced, unfortunately without a warranty.  However, the manufacturer will always provide a replacement implant (or two) at no charge for a leak.  So you should get a little price break for this surgery.

What Size Implants Would Be Best?

One breast is a small C while the other is an ok sized D

A: Different size implants, may need a lift if larger side is saggier

You obviously need different sizes.  Frequently the side that is larger is also saggier and the nipple is lower on this side.  Is this the case?  If so, you might need to have a simultaneous breast lift.  I am presuming you want to be a full D cup size, with a small implant in your larger breast and a larger one in your smaller breast.  It is impossible to tell you the size without photos and seeing you in person.  It is still an art when it comes to implant size selection.  You are best advised to view photos of your surgeon's work, particularly in patients with breast size asymmetry.
