If the bottom of the chin is burred down vertically, can this be done under local anesthesia?
Based on your level of anxiety and tolerance of discomfort, it is possible to do a chin procedure with local anesthesia (and most likely a nerve block similar to when going to the dentist). You may ask your surgeon if a pill sedative would be appropriate also.

If you are tolerant to the local anesthetics used to achieve the block, performing a chin rasping or burring is feasible under a local anesthetic.

I think an augmentation genioplasty with a sliding advancement osteotomy or implant would provide a substantial difference in your appearance with pleasing result.
