I Have Double Eyelids but Want to Get Bigger Eyes--what Kind of Surgery Do You Recommend?

I'm Asian and want bigger eyes, but unfortunately I already have double eyelids. Is there anything I can to make my eyes look bigger, or are there any surgeries available?

A: Bigger Asian eyes by design should look naturally beautiful

Your photos suggest constriction by the medial epcanthal folds.  I recommend a release of the epicanthal folds to widen the horizontal axis and this will balance your almond shapes nicely.  Although I can raise your folds, I don't recommend you change the pretarsal folds as they are already at the top of the natural balance point for Asian eyelids.  Beyond this, the folds may appear "surgical".

Does Asian Blepharoplasty Cost More Than Non-Asian Blepharoplasty?

Asian blepharoplasty cost more than non-Asian blepharoplasty? I am an Asian with a crease want remove some excess fat and skin. Thank you.

A: Asian blepharoplasty

Although the techniques are different, and do require more precision, the costs for Asian blepharoplasty are generally equivalent to standard blepharoplasties.  I can offer a less expensive "no cutting" stitch technique for selected patients who do not need excess skin removal.  This is a modification of a minimal access, no scar technique pioneered in Japan.

Epicanthal Fold Type THREE to Type ONE

I have epicanthal fold type 3,I want to become #1. Yes,I realize this will have a westernizing effect and may potentially make me look to Caucasian, but I still want it, depending on the risk of SCARING. WHat are my chances of suffering unsightly scarring? I realize unsightly is relative, but what you, as a surgeon, woudl consider acceptable or unacceptable, and would it be hypertrophic or just hyperpigmentation

A: Epicanthal release will give a larger eye opening

The successful conversion of a type 3 epicanthal fold to type 1 depends on your 3D orbital-nasal bone structure and the availability of the levator support at the medial portion of the upper lid.  The drawings you post are oversimplified skin diagrams that can be useful for planning, but in reality, will not be able to predict results reliably.  Hyperpigmented scars are usually temporary and minimized with proper sun protection.  Hypertrophic scars are rare with proper use of nonabsorbale suture (less inflammatory) removed at 3 to 5 days.  Best to ask your surgeon to show you a variety of results with the effects you seek.
