At 66, what would be the best treatment for lower face? I have read so many negative articles about thread lift. Fillers will not be enough; I need a lift. Do I need a lower face lift or is there an easier way to fluff this area up?
Tightening of the skin of the neck and lower face can only occur with a well executed lower facelift and necklift combination. Skilled Plastic Surgeons should be able to perform this surgery for you. Fillers or lasers will not accomplish this. Trust your surgeon.

The pictures make the results look pretty similar. What are the differences in operation?
Aging is not restricted to only the neck area, so it would be unlikely to only need a necklift without the incorporation of the lower facelift as well. As I perform the surgeries, I include the lower facelift with my necklifts for most of the patients. Excess skin tightening as well as better muscle tone should include elevation and lateral rotation of the platysmal and SMAS layers with the neck and lower facial skin.

when my face is in a neutral positionI have baggy cheeks that looks saggy from nose down. When I smile my cheeks lift up and i then have a heart shaped face but when in neutral position it is more like a square it looks like I have more cheeks on my lower face than on my cheek bones but I cannot say I have flat cheekbones either.what is the best solution to treat that? I have tried fat injections before which helped with the cheek enhancement around the bone, is a mid face lift solution for me?
I don't believe you need a mid cheek lift. Fillers should give you the suggestion of a fuller face without resorting to a mid cheek lift. The lift may exaggerate the deformity and continue to give you too much fullness. Sometimes fillers are more appropriate for the patient, and I believe this may be one of those times.
