Will Evaluation for Lower Facelift Be Accurate if I Have Fillers?

I just had Radiesse for my nasolabial folds and Juvederm to fill out the upper and a little for the lower lip. Nice improvement. However, the little jowls are still there and since fillers or lasers won't help, I'm afraid that some sort of lower lift is in the near future. My question is, can a surgeon evaluate my face properly with all these fillers? Will I need to let the fillers be absorbed before surgery?

A: Fillers and facelifts

I agree that an experienced board-certified plastic surgeon should be able to at least estimate the influence of your filler material volume on your needs for skin tightening. I think it is prudent to wait 6 months or so from the time you had the fillers; while the Radiesse (which feels firmer) may remain, he Juvederm may be reabsorbing at that point, making it a little easier to judge the volume deficiency.

Improving Marionette Lines with Lower Facelift?

How much improvement can I expect from a Lower facelift with fairly significant marionette lines?

When I hold my face where approximately I think it will look after surgery, I still see the lines. They become less severe, but somewhat horizontal. How often are some type of fillers needed with the surgery? Thank you.

A: Marionette lines

Marionette lines may respond to fillers like Juvederm or Restylane, or more permanently to fat transfers. The lower facelift should help redrape and tighten the skin of the jowl area, and depending on the vector of pull, may help correct the marionette lines well. These procedures are often combined to optimize results. A consultation with a board-certified plastic surgeon can help you in your decision process.

Sagging Jowls - Is a Mid-Face Lift Needed with Lower Lift?

I am 54, but look slightly younger. I don't mind the normal aging around my eyes, etc. but my sagging jowls bother me. My Plastic Surgeon says he would not recommend having a lower face lift without at least also doing a mid-face lift, as well as a brow lift. Now he has agreed he would feel comfortable doing the lift without a brow lift, but says he would definitely recommend a mid-face lift with the lower face lift. Is it really necessary to also have a mid face lift with a lower face lift?

A: Lower facelift

What really distinguishes a "lower facelift" from a standard facelift is how high into the cheek and temple the surgeon dissects. Some surgeons use the term lower facelift to imply a lesser degree of complexity or downtime to the surgery, which I do not always think is an accurate representation. Depending on how hollowed out the area of the cheekbones is, you may need some form of suspension of the mid-facial tissues beyond the lower face/necklift territory, which in turn may require a longer incision or work on the area of the temple as well. A comprehensive consultation with one or two additional board-certified plastic surgeons should help you focus your decision and come up with a plan with which you are comfortable.
