How to Give my Face a Contoured, Feminine Shape?

I am 21 years old, in good health (5'4 at about 120-125 pounds) and face is too round and has no real shape. I already had a rhinoplasty and chin implant which helped my profile but not my frontal view in the least. I have no double chin or excess fat on my neck but there is excess fat around the jawline/lower chin/jowl area. Can liposuction help? What other procedures would you recommend for me to give my face a nice shape? Buccal fat removal? Jaw implant? Sliding genioplasty? Thanks.

A: I would caution AGAINST Buccal Fat removal

Having a full face and round cheeks is a sign of youth.  Simply do a bit of research to see what concerns women in their 30's and 40's are looking to correct...Volume Loss is at the top of the list.  You will find that over the next 5-15 years your cheeks and lower face will thin and you will have the desired (more chiseled) look you are wanting.  If you choose buccal fat removal in your early 20's, you may be happy with the result for the next few years- BUT you will be faced with a more aged and thin look in your face in your early to mid thirties.  I have a patient in my practice that is facing this exact issue.  At 35 she is attractive, youthful and vibrant...but her face looks a bit tired and "deflated".  She had her buccal fat removed by another surgeon when she was 22 and now, at 35, she is seeking to volumize her face with fillers such as sculptra and radiesse which will cost her approximately $3,000-$5,000 every 5 years.  Although this may not be the answer that you are looking for- you may find that a visit to a professional make up artist to learn the trick to contouring facial features with blush and bronzers will get you through the next few years.

Best Options for Sagging and Droopy Face?

Hi, I am a 25 year old female, 5'6" about 130lbs and am very upset with the draggy and saggy way my face has begun to droop. I have had a couple of procedures advised to me and I am now kind of confused as to what I'd like.

I would like to get fullness back in my cheeks and take care of my folds around my mouth also the asymmetry by my eyebrows. I have been given the options of fillers and buccal fat removal or a Mid facelift. What should be the better procedure?

A: Dermal fillers versus a lower facelift

Patients today have many options for facial rejuvenation of the lower face. Dermal fillers are extremely popular impatience I want to correct the nasolabial folds and marionette lines (otherwise known as the smile lines). If this is your main concern, a dermal filler may do the trick.

If you're interested in changing the shape of the jaw or in redraping the skin on your neck, a lower facelift or in your case, a midface lift would be more appropriate. Your best bet is to work with a board-certified plastic surgeon who has great deal of experience in lower facelift & midface techniques, dermal fillers, and facial rejuvenation. They will be able to assess your concerns and recommend a course of treatment that will meet your personal goals.

Lower Facelift with Vertical Skin-pull Vectors?

Is it at all possible to get a lower facelift without using a horizontal vector? Are there any lower facelifts that use non-horizontal/vertical vector for skin repositioning? I'm a bit terrified of having my lips elongated horizonatlly (as one plastic surgeon was kind enough to simulate for me)--please tell me there are other ways of going about this!

A: Lower face lift effect on lips

A lower facelift is a powerful and effective procedure to recontour the lower face, the jaw, and neck. During this procedure, the deep tissue of the neck is identified, the excess fat is treated, and this tissue is resuspended to be stronger tissue around the ear and skull. The vectors used for the resuspension are important. These vectors should be more vertical and NOT horizontal in order to prevent the windblown, tight, overdone look of a poorly done facelift. With proper repositioning, this sugery will not affect the appearance or position of your lips or the corners of your mouth.
