I'm currently halfway through chemo for breast cancer; the plan is for bilateral mastectomy afterwards. I'm quite intrigued by the total fat transfer approach (sometimes used in conjunction w/Brava system) & would love to find someone on west coast who has experience doing this. I'm reading so many negative comments on the online discussions about tissue expanders, any kind of implants, ripples, unevenness, etc. Very unhappy women! 100% fat transfer might help avoid these issues.
I am a firm believer in breast reconstruction with fat, and have just completed bilateral mastectomy reconstruction with only fat on my own wife. On the West Coast I would recommend you consult with Dr. Joel Aronowitz in LA. BRAVA is essential for the most efficient reconstruction, allowing reconstruction in 3-4 procedures vs 7-8 procedures. My wife and I can speak first hand regarding this procedure and the BRAVA etc if you like.
My ps does the nipples at the same time of the exchange surgery, from the expanders to the silicone implants. Is this ok? I know my surgeon has a great reputation, but I've heard most Dr's do the nipples at another time. What are your feelings on this? And what input do I have as far as the ending results of the size and shape of the nipples? Meaning, is this totally in the Dr's hands?

It is certainly possible to reconstruct the nipple at the same time as the implant placement, but in my opinion this is much more likely to result in asymmetry or a result you are not pleased with.
After your implants go in, there will be a period during which your outcome will "evolve", with your breasts changing in subtle ways. My preference would be to not put the nipples on until this has occurred.
Ater having a nipple sparing mastectomy on my left breast, I am trying to figure out what breast implant size would most equal my reconstructed breast. My reconstructed breast is a Natrelle Silicone style 45, 460cc, 11.4cm width, 5.9 projection. My right side is a Natrelle silicone style 20, 375cc, 11.7 width, and 4.9 projection. My right side implant appears too small. Before surgery and augmentation, my bra size was a 34 A cup. What size implant on my right side will make me more even.

I would have to agree with my colleague from New York...
This is a decision you will need to trust your surgeon to make, and both your outcome and your relationship with him will be adversely affected by inserting a bunch of opinions from doctors who have neither met you or been involved in your care to date.
Trust is important in good medicine... You've come this far with your surgeon- unless you have reason to not trust him now, follow his recommendations and instructions.
