Direct sutures or suspension (NOT thread) sutures into the SMAS? What has been your experience long term?
My opinion is that no technique is as effective and capable of creating a natural appearance as selective SMAS dissection and suspension. The decision to conserve or trim SMAS is made on an individualized basis. I see imbrication and plication without dissection as a shortcut that dampens the response to the intervention. That being said, an excellent surgeon with a different preferred techniqaue will deliver a superior result to a poor surgeon using the best technique. I think its best to find a sureon that you trust and respect and let them guide you. An attachment to technique and pull you away from what is really important!

I had my SMASS facelift (and upper eyelids) done 4 days ago. My cheeks are very swallen and hard, and I wonder for how long is it helpful to use ice pads to reduce bruising/swelling. Can the skin stretch considerably from prolonged swelling? One side of the neck is very bruised, but not swallen like the cheeks. I am 49 y.o.
From the pictures it seems that you have normal amount of swelling after a Facelift and upper eyelid surgery.
Ice usually helps with swelling in the first 48 to 72 hours (do not use directly on the skin since after a Facelift the nerves are severed and you have no sensation over the skin). After that period of time icing does not change much of the swelling.
You may use black and blue ointment or any product with Arnica and Bromelin.
Sleep with your head elevated.
It takes about 4 weeks on average for a significant amount of the swelling to go away.
My swelling was much worse on left side after surgery to begin with and then I developed an infection in left ear incisions and had to take course of Cipro. The skin in front of my left ear is still more swollen and numb and numb than the right but not enough for anyone else to notice. What is visible is how swollen my left earlobe is. My right earlobe looks completely normal. Could the facelift also have made my left earlobe swell and will it resolve in time?
Your signs and symptoms are not at all unusual even 2 months post op facelift. Swellingo of the ear can take a few months to resolove fully (remember, there is an incision around almost the entire ear!), especially if one side was particularly swollen early post op. Also an infection or suture irritation can result in prolonged or recurrent swelling. Be sure to follow up with your surgeon. Ultimately it will all resolve and if there remains an earlobe asymmetry that is bothersome, it is usually an easy procedure to correct.
