I had a punch biopsy done to remove a mole from my leg. The diagnosis was: "lentiginous junctional dysplastic nervus with moderate atypia, focally extending to the peripheral specimen margin". The doctor told me that the result is completely normal and that I don't need to have anything else done. Is is true that no other surgical excision is needed? Thank you very much.
Although biologically benign, most doctors agree that it is best to remove dysplastic nevi. A punch biopsy is only used for diagnosis but not treatement unless the lesion is so tiny that it is completely removed with the punch and does not come to the margin.
I had a punch biopsy done for a mole and the result came back as displastic nervous with moderate atypia extending to the peripheral margin. The doctor said I should have more removed ( re-excised) but he wouldn't do it now, since I am 5 months pregnant and I just have the sutures removed ( from the punch biopsy). He said that it is not urgent and I should have it done after delivery. Is it true that I can postpone it a few months ( without anything bad to happen ). Thank you very much.
A dysplastic nevus is a biologically BENIGN lesion. Since it is not a malignancy or even pre-malignant, re-excision can be delayed indefinitely. The only reason for re-excison is because of an association between having multiple dysplastic nevi and melanoma. There is no proof that a single dysplastic nevus will become a malignancy. So the answer is, you can absolutely wait to have the excision after your pregnancy and breast feeding.
Is laser removal a possibility?
Mole removal is best accomplished by surgical removal, rather than laser for both cosmetic and medical reasons because with surgery, a biopsy is obtained. However, in the central cheek there should always be concern about the cosmetic outcome particularly in African Americans where keloid formation may be a hereditary problem, and subsequent treatment of the keloid may be problematic due to pigmentary issues.
