I have consulted a board certified PS & am in process of scheduling the surgery. Dr. has said for perkiest look go for a full lift w/ augmentation, and for a more natural look go with a donut lift w/ augmentation. Below are photos of the look I'm after & my own breasts. I know "perky" and "natural" have different definitions depending on each person. Which lifting would bring mine to the same position and shape? I know all breasts are different, I am simply going for that "style" in the pics
I would recommend a technique called circum-nipple mastopexy with cold-subfascial augmentation. you have large areolas and with this technique you can have completely hidden scars instead of scars at the areola-breast junction. With the cold-subfascial technique the breast implant is supported into a tear-drop shape like the result you are after. The cold-subfascial technique also prevents the dramatic 'muscle jumping' that you can expect with subpectoral and dual-plane techniques. Even in the best hands a Benilli lift tends to flatten the breast and always results in widened unattractive scars with time. If you are considering Benelli lift you would be much better off with a vertical lift as it offers support and does not typically result in a widened scar. I hope this helps!

The 'secondary' surgery (second procedure that must consider the changes incurred by the first operation) is certainly more complicated than the original surgery and choosing the correct approach is critical to success. I believe that you will absolutely require a 'full lift' to improve your breast shape / position. The exact length of the incisions required to remove excess skin will be determined at the time of surgery, after a new (smaller) implant is placed and the 'pockets' / capsule space is altered internally to improve the implant position. The capsular tissue as well as breast tissue is restructured internally to support the breast and implant, the skin is then 'redraped' (not used for significant support). Therefore, careful attention must be paid to restructuring the internal components ( breast and capsular tissue) and removing the appropriate amount of skin to both reposition the nipple and reshape the breast.
I am 33 years old and done having children. I am considering having a breast lift done but am concerned with how they will turn out after surgery. Ideally I would like them to be lifted and rounder. I am currently a size 32D-DD and dont feel like I want to add more sizewise, but I worry that if I do not add an implant that I will be left with flat looking breasts. I would like to not have the T shapped/lollipop scar, is that an option for me? Will I need an implant?
Thank you for the photos and questions, Sherrie. I agree with completely. You do not need and should not have a combined mastopexy (lift) and augmentation. You will gain some superior fullness and projection with a vertical lift (less with a Wise pattern). The reason you should not get implants at least initially is studies show that raising your nipple more than 4 cm and then placing an implant at the same sitting can lead to increased complications. I think you will be happy with a lift alone.
