Now 9 Months PO, Noticed "Bottoming Out" at 4 Mo PO, I Approached my Dr. and He Totally Denies This. What To Do? (photo)

Had BA 4/11, had bottoming out 4 mo PO with stretch marks. I am now 9 mo PO couple months ago had PO visit surgeon denied this, but did not take measure. or allow me to lie down to show him, he insisted on the 550 cc. When I lay down they're at my armpit,even with a bra no cleavage.He blew me off, told me i am just an unhappy person told me to come back 3 mo, I've consulted w/ another Dr. who totally agreed, but don't feel i should have to pay $9k to fix.My next PO visit is March.What to do now?

A: Bottoming out versus asymmetry to begin with.

Based on the preoperative photos you provided, your folds were uneven to start with and you had a degree of asymmetry with more overhang of the left breast. The right breast fold is lower than the left. Unfortunately, the photos from 1-2weeks and 4 months are a bit deceiving because of the arm position and how it changes the appearance of the breasts. Still, you have very large implants and you have some bottoming out or "pseudoptosis" in both breasts. The cleavage issue has more to do with your broad chest and breast position than the implants. Placing the implants more medially will rotate your nipples to the side, which you likely would not be happy with. I am sorry to hear of the issues you are having with your plastic surgeon. Your options seem limited to using a different surgeon or keeping things as they are. If you have developed a hostile relationship with the first surgeon, I would not recommend a reoperation by that person regardless of financial incentives.

I Am Wanting to Have Large Implants Exchanged for Smaller Ones, What About the Risk of Loose Skin?

i am 5'2 and have implants which were overfilled to 350 cc. i have been measured and told i am 32DD. i feel this is too large for my frame and want to be a small c. my questions are 1)how much less cc's do my new implants need to be approximately to achieve my desired cup size? 2) i have good skin elasticy but will i likely have loose skin? 3) besides reducing the pocket is there anything else that can be done to help appearance? i have had these implants for 11 yrs.

A: How to choose smaller implants and decide on a lift.

The most straightforward approach to determine your lift and implant choices are to remove the current implants from the clouding the picture. For saline implants this can be done with a deflation procedure. This procedure will eliminate the volume of the current implants, allow the breasts to show the degree of looseness or ptosis, and allow you and your surgeon to more accurately predict which implants will give you the look you desire and whether a lift is required.

Subglandular Saline Implant Deflation in Doctor's Office, What Happens to Shell Until Removed?

Considering implant deflation in the office. What happens to the shell after saline is removed since my implants are NOT behind my muscle? Will they just lay flat until I decide to have them removed or shrivel up and be noticeable? I wanted to explant but found out I'm pregnant so doctor offered to deflate and then remove after my pregnancy.

A: Saline shell after deflation.

The shell may be palpable as it is folded up inside the pocket. Typically, it's not visible. All of this depends on the amount of breast tissue you have overlying the implants of course. Have your surgeon show you a deflated saline implant to give you an idea of what they look like when they are not filled.  This alone may answer your question.
