I am now 4 months post op from lower blepharoplasty with a skin pinch incision that left me with ectropion / retracted lower lids. What procedure best fixes my problem -- canthopexy or canthoplasty with cheek lift? Would restylane help boost my lids up or will I need surgery? Also, seems most oculoplastic surgeons who specialize in such repairs are in CA. Any recs for Chicago doctors who routinely fix this issue? I can't risk another failed surgery so would travel to the best. Many thanks.
Your photo shows a slight ectropion on the right only. At 4 months it may still be possible to massage and stretch the lid up a bit at the corner. If that is not successful, it may be best to consider a revision surgery for which likely a canthopexy would be sufficient and the least invasive of the three surgical options you mentioned.
Restylane injection would not likely be successful at improving the problem.
You should consult with your treating surgeon first for recommendations. If he/she does not provide the reassurance/solution that you are seeking, then a second opinion is very sensible.
I had lower blepharoplasty because I had a little extra skin under my eyes. I chose an experienced plastic surgeon.He told me that he would pull the muscle up and I would be very happy with results. After surgery he informed me that he did not remove any skin. By pulling the muscle the skin would also get firmer. I am in a panic because Monday, Nov. 22 was 3 weeks since surgery. The skin is not firmer but I also have deep lines and hollow circles under my eyes that were not there before.
You are understandably anxious about your appearance, but it is best to be patient. At 3-4 weeks, you definitely still have swelling, eyelid function has not fully returned yet and you will see some improvement. It is important to keep your eyes well-lubricated with eye drops and to gently massage the lower eyelids upward. Paper tape can also be applied to hold the lid up when you are home.
You should discuss your concerns with your plastic surgeon so he/she can advise you regarding further early postop care and as to whether any future treatments/procedures will be needed.

Are some conditions resolved with non-surgical alternatives to blepharoplasty? How often do patients end up needing surgery in the long run anyway?
Non-surgical alternatives for blepharoplasty include Botox/Dysport injections to raise the eyebrow which can result in a more open eye apperance, fillers (Restylane or Juvederm) to carefully smooth out the under eye area and chemical peels or laser to smooth fine lines and imnprove pigment. All of these effects are temporary and therefore proceeding with a surgical treatment can be considered for a more long-lasting result.
