What is the Best Implant Position for Tuberous Breast? (photo)

I am 27 yrs old & a 36B. I have 1 son who I nursed. I am very athlethic. My PS says that I have tuberous breast & he will have to give me a mastopexy/augmentation. What is the best placement for the implant? Submuscular or subglandular? He says that the subglandular is better considering my constriction but I have heard that submuscular is the placement of choice when it comes to longevity, future ptosis and rippling/contracture. I want to go up to a full C/small D.

A: Tuberous?

I do not see a tuberous breast but rather a ptotic(droopy) one.   I agree that you will require a lift to achieve optimal results however I do not believe that a breast implant should ever be placed above the muscle.  The risk for long term problems with this method is much greater and I do not believe surgeons who specialize in augmentations would do that.  Insist on going submuscular.

I'm in a Lot of Pain After 9 Months Breast Implant, What Could be Causing this?

On may 3, 2011 I had a second breast surgery from my left hand side. Some times I'm on so much pain.This pain come and goes.I'm taking 800 milligrams of ibuprofen to feel better. Some times came from the back, some times is from my nipple and if I push it hurt a lot. I had a mammogram and sonogram on June 2011. It came clear. I went to doctor and he send me for MRI and x-rays.I don't feel anything hard or round when I check myself.Sometimes it is a weird pain like a contraction.I'm scared.

A: It depends

A complete history is needed to sort out your problem. In general, alot of pain 9 months after breast surgey is not a normal sign.  Typical causes are capsular contracture and infection, unless there was trauma.  You mentioned a mammogram performed 1 month postop.  That would have been very early and certainly could have caused some pain.

You should follow up with your surgeon or if he/she is not giving you the answers/reassurance you need, then consider a second opinion.

Do I Need a Breast Lift or Only Implants? (photo)

I was overweight when I was younger and have lost 55lbs since. My breasts were never large to begin with and at this point have lost even more volume and are saggy which makes me extremely insecure. Once I reach my weight loss goal (15lbs to go) I plan to get my breasts augmented though I am not sure if I only need implants or if I should get implants with a lift. Any recommendations?

A: Both procedures may be needed

Good for you that you have lost weight,  that you have a goal weight and that you hope to achieve that before surgery.  There are many variables including weight fluctation that can affect breast size and droopiness, therefore it is a good idea for your weight to be as stable as possible prior to proceeding.

You would need a formal examination to determine the best procedure for you and to hear all of your options, but from your photos, it appears that augmentation will be your primary goal.  Additionally a lift will be needed for the "sagginess" and to help with the asymmetry (your two breasts are different which is of course not uncommon). 

Good luck and consult with a plastic surgeon to address all of your concerns.
