Thoughts About Having Cosmetic Surgery (Face Lift) at a Residency Program?
Should you have cosmetic surgery at a residency program? This is a great question. The answer depends upon the surgery you choose and who will be performing your surgery. At most university hospitals, you can choose a board-certified attending plastic surgeon to perform your procedure. This is the person who trains the residents and fellows. If you choose to have cosmetic surgery with an attending surgeon at an academic hospital, you should ask who will be performing your surgery. Resident and fellows will likely "assist" their attending during surgery with retraction, etc. Different surgeons will have different levels of comfort with having residents or fellows do more than just assist. If you do not want a resident or fellow performing cosmetic surgery for you and want them only to help the surgeon you should clearly state this. If your surgeon agrees to this, you trust them and they've demonstrated great results with pre- and post-surgery photos, then you can proceed with confidence.
Thank you for your question.

I've had several consultations. One doctor puts in drains, one doesn't, and one uses glue.
Placement of drains or use of tissue sealant is surgeon specific. Do you need either? No. You have walked into the reality of facial plastic surgery which is that there are many ways to perform each procedure. Neither drains nor tissue sealant are required for any type of facelift. What is important is that you find a competent, trustworthy Facial Plastic Surgeon who has post-operative photos that are pleasing to your eye. Whether they use drains or sealant should not be a deciding factor. We do things the way that we do them because that is the method that has worked in our individual or small group experience. You want a surgeon that achieves great results even if it requires Tisseel and a couple of drains to get that result.
Thank you for your question.

Does face lift threading produce the same results as a traditional face lift?
Threadlifting involves the use of permanent, barbed sutures that are "treaded" under the skin. They are placed in precise positions and the skin is pulled in an upward direction to counteract the effects of aging. Like every procedure complications ar possible, including Infection of the threads and extrusion of threads (working their way out through the skin). Perhaps the biggest downside to threadlifting, however, is the lack of longevity. Typically, the effects of a threadlift only last a few months, and yet the cost for the procedure is not insignificant.
Facelift procedures, on the other hand, enjoy great longevity. The outcome from facelift procedures also tends to be superior to threadlifiting, especially in the jowls. This improved outcome does come with its own set of potential complications, including fluid collections under the skin and numbness of the earlobe. There is also a greater potential for poor scars in inexperienced hands. While these risks are not insignificant, most patients enjoy fantastic results and are generally very satisfied.
