I have saline subpect implants I want out for several reasons. I want a simple extraction. What are the odds that I will have fluid accumulate and do I need a drain placed if the capsules are left in place?
I tend not to use them in uncomplicated cases and over time the pocket will shrink up. If there was an infection or issues with the capsule such as calcification then more surgery may have been necessary and drains likely would be helpful.
I had my implants 4 years ago, under the muscle. I went from an A cup to a B cup. But I never really wanted them in the first place. I want them removed my plastic surgeon who place them says that he can remove it with local anesthesia (lidocaine) in the medical office. I will be wide awake. Is that safe, will I feel the pain?
The simple answer is yes but the more complicated answer is, it depends. Simply removing your implants is straightforward but many of us will also irritate (or remove) the capsule surrounding the implant so that your long-term result is more natural from the both standpoint of look and feel.
I would suggest having it done under some type of anesthesia so that you can get the best result possible. Short cuts don't often yield the most optimal results in the long run.

At 18 & a 32C I got implants to a full D. Now 33 with 2 children I am a 32F. I want to have my implant removed, would you recommend a lift? Also would it be difficult to guesstimate my size after the removal? Thx
I would agree with deflating your saline implants in the office, waiting 4-6 weeks, and then seeing how much the skin has retracted on its own. In many cases, unless you currently have very large implants or your skin is very loose, you may be able to avoid a lift.
As for your final cup size, there is very little that anyone can do to accurately prdict this before surgery.
