Breast Aug Scars - 4 Months Post-Op

My scars are hypertrophic and unsightly. I am overall pleased with the breast aug but the scars make me feel like I've swapped one set of issues for another. I am using silicone strips but am not satisfied with the results. My surgeon is sort of dismissive about them and just want me to wait it out. Do most surgeons provide kenalog/cortison shots post-op or is it common to have a surgery and then have to find another doc to take care of the scars? Please post suggestions for my scars.

A: breast augmentation scars

Your plastic surgeon may be overly dismissive but the reality is that there is little that can be done to improve hypertrophic scars or speed up the maturation process. Pressure can soften scars and speed up maturation but it's unrealistic in this location. Steroid injection will speed up the maturation process but won't change the width, length, position, or color of the scars. I would condemn the idea of laser treatment and scar massage or topical treatments are also wishful thinking. Occlusion with something like silicone sheeting may help speed up maturation but won't really change the outcome. 

The best that we can do is to limit the length of the incisional scar to the minimum, control the location of the scar, make sure the patient understands the scar tradeoff, close it as well as can be done and minimize healing problems, and avoid irritation to the scar while it is trying to mature. In your case I would agree that the implant may have been too wide for the position it was to fill, the incision rose up on the underside of the breast, and the length of the incision does not appear to be the minimum that can be done today (about 3 1/2 cm with a Keller funnel for silicone gel implants -- 2 cm for saline-filled implants). Unfortunately there is nothing you can do to erase the scars and start over so you will have to consider steroid injection or allow full maturation over a year or more.

Acne Scars on the Back and Shoulders?

I suffered from back acne some years ago. My back and my shoulders have now, plenty of white, little scars. They are sort of kelloid because they have volume but they’re not big, still ,I don’t like them. I would like to know what the best option would be to get rid of them.

A: Acne scars on shoulders and back

Raised scars from acne can be treated by resurfacing. The best option would be laser resurfacing with a fractional laser. Your skin may have to be pre-treated with a bleaching cream to avoid any hyperpigmentation from these treatments depending on your skin type and color.

Chemical peels may help reduce your raised scars as well. A series of treatments is recommended to get the best results, and results may be gradual. Total resolution may not be achieved, but the appearance of your skin should improve with completion of a series.

Avoid direct sun exposure to reduce the appearance of discolored scars and wear a broad-spectrum sunscreen of  SPF 30 or higher when the skin is exposed to the sun.

I recommend a consultation with a dermatologist and/or skin care specialist to assess your condition. They will then recommend the best treatment course for you. Good luck!

Surgical Scar on Arm - Most Effective Procedure and Cost?

I had microsurgery and had my tendons fixed after an basketball injury involving a garage window 5 years ago. There is a scar on my arm from the surgery that still haunts me...What is the most effective method or surgical procedure that would have the best results for reducing/erasing my scar & also what price range. Thanks

A: Some Scars Can't Be Improved

Scars can be an unpleasant reminder of past traumas or dreadful events.

That association can be a strong motivation for scar revision.

Unfortunately, scar revision techniques are limited: the scar in your photograph appears to be about as good as could be achieved.  I would probably not advise any further procedures, as you run the risk of making the scar worse, rather than better.

You could try a test spot with a laser treatment to see if that could be beneficial, but I think you are not likely to make the scar any better.
