Do I Need a Lower Facelift?

I'm 44 and have noticed the oral commissure starting to drop. I have considered filler in this area but I'm not sure which one is better. I've been told that I need a lower facelift to eliminate jowls and tighten my neck, yet another consult told me I just needed filler. I just had a 20% TCA peel done 2 months ago along with IPL, Fraxel and Accent. I see no improvement from any of these modalities. I'm not sure what to think at this point because I'm not sure what I need done. Thank you.

A: Start Facial Rejuvenation With Small Steps

You might consider starting with a hyaluronin filler, like Juvederm Ultra Plus, for the creases around the mouth and fine lines of the lip. Restoring volume or judicious plumping can produce remarkably gratifying results.  The fillers generally last about one year.

After trying the filler, if you are looking for more or different results, a Facelift (technique to be determined: mini, lower third, etc.) might be your next step.

A: Early signs of aging

You are displaying the early signs of aging. You have several options available depending on what you want to achieve. Early jowls can be treated with a lower minimally invasive facelift (minilift). The results will be there, but subtle. Another option is fat transfer to your developing marionette lines, nasolabial folds, and the the area just in front of your jowls. The prejowl fat transfer will provide you with a more smooth and continuous jawline and buy you time until the jowls descend more.

You may consider fillers first and see if you are happy with the result. If you are pleased, then that would have served as a preview of what you may achieve with fat transfer.

Can Jowls Come Back After a Lower Face/neck Lift?

Is it true that jowls can reappear just a few years after a lower face/neck lift? i know a few women to whom this has happened. They are older than I am, and had more jowling to begin with. I am not yet 50 with very little jowling and neck skin. Can I hope for a longer lasting result?

A: Unfortunately, there is no way to completely halt the aging process; the jowls wil eventually recur.

The goal with all facial cosmetic procedures is to look and feel natural, refreshed, and younger by setting the clock back a few years. Unfortunately, there is no way to completely halt the aging process. Even after following a face and neck lift, the signs of facial aging will reoccur. On the average, a face and neck lift should last about 5-10 years. This is dependent upon the surgeon’s skill and technique, your personal genetics (physical characteristics), and skin care maintenance. I strongly recommend that you have a thorough consultation with a board certified plastic surgeon or facial plastic surgeon experienced in facial rejuvenation.
