Recommendations for Tired Eyes?

I'm 38, I have pictures attached, I have sun damage and a tired look. I'm really bothered by my tired look under my eyes, and I want my laugh lines filled in and my upper lids addressed.

My biggest concern though is the lower eye and tired look! I have had 4 consults and was told 3 different things from under eye injections, MiXto Laser to surgery. Please look at my pics and advise me as to what procedures you'd recommend.

I'm thinking surgery, but there are other issues with my face I want to address to look a little younger and fresh. Thank you so much for your input! Just trying to do the right thing.

A: Improving a tired look

Sometimes the "tired look is not related to lax skin or extra fat in the lids which what we commonly address in eyelid surgery.Early aging changes such as yours often manifest as circles beneath the eyes and a flattening of the midface. This is caused by a loss of fat in the infraorbital region leaving thin skin over the orbital rim. When one looks at very young people they usually have upper facial fullness and you can not see a delineation between where the eyelid stops and the cheek starts.

Correction therefore is based on restoring the volume. I feel this is best done with a fat transfer but in some cases fillers may work well. I seldom do a facial surgery that is not enhanced by volume restoration and in younger patients that may be all that is needed.

Eyelid Surgery Revision Possible with Chemosis Present?

I had eyelid surgery 5 months ago. It went bad and now I need to have it redone. My new plastic surgeon told me that I have chemosis in one of my eyes. I am supposed to have my new surgery soon to repair what my other surgeon messed up on. I want to know, can it be redone even though I have chemosis? Or will that worsen the problem?

A: Chemosis and surgery

The definitive answer depends on the problem for which you require revision. In some cases, ectropion or eyelid drooping may be contributing to the problem and a permanent or temporary correction of this problem could be warranted before complete resolution of chemosis. You should sek consultation with a plastic surgeon or opthalmic plastic surgeon with specific experience with this problem.

Risks of Fat Pad Removal on Lower Eyelids?

When they remove the fat pads from the lower lids is there a risk to tear production?

A: Tear production after lower lid surgery

Lower lid surgery normally does not interfere with tear production. However, temporary dryness of the cornea can occur. Skilled surgeon's pay great attention to this potential problem and if it presents it can be treated with lubrication and other conservative options in the vast majority of cases. This problem is more likely to occur in those with a previous history of dryness or inability to wear contacts. In these individuals it is wise to test the tear production before surgery.
